摘　　要：基于史密斯模型,从政策本身、社区数字化慢病管理执行者、社区数字化慢病管理接受者、环境因素四个方面探讨社区慢病管理数字化转型的实施困境,建议政府加强顶层设计、鼓励多主体参与、调动社区医务人员和居民的参与积极性,充分利用一切有利因素,推动社区数字化慢病管理发展。Based on Smith's policy implementation process model,the implementation dilemma of the digital transformation of community chronic disease management is discussed from four aspects,the policy itself,the implementers of community digital chronic disease management,the recipients of community digital chronic disease management,and the environmental factors.It is suggested that the government should strengthen the top-level design,encourage the participation of multi-subjects,mobilize the participation of community medical personnel and residents,and make full use of all favorable factors to promote the development of community digital chronic disease management.
