序贯血液净化治疗急性重症有机磷中毒患者的临床效果及对心肌功能的影响

摘　　要：目的分析序贯血液净化治疗在急性重症有机磷中毒患者中的效果。方法选取2021年4月至2022年4月寿光市人民医院血液透析室收治的82例急性重症有机磷中毒患者作为研究对象, 按数字表法随机分为两组。对照组41例, 给予常规内科治疗+血液灌流, 观察组41例, 在对照组治疗基础上加以序贯血液净化治疗, 连续治疗3 a。比较两组患者临床疗效、恢复情况和心肌功能。结果观察组患者治疗总有效率高于对照组(P<0.05)。观察组患者血清胆碱酯酶活性恢复时间、昏迷时间明显短于对照组, 阿托品用量明显小于对照组(P<0.05)。治疗前, 两组患者肌酸激酶同工酶、肌钙蛋白T、肌酸激酶、乳酸脱氢酶比较, 差异无统计学意义(P>0.05);治疗后, 观察组患者的肌酸激酶同工酶、肌钙蛋白T、肌酸激酶、乳酸脱氢酶明显低于对照组患者(P<0.05)。结论序贯血液净化治疗急性重症有机磷中毒患者效果显著, 能够保护患者心肌功能, 减少心肌损伤, 加快症状缓解。ObjectiveTo analyze the effect of sequential blood purification therapy on patients with acute severe organophosphorus poisoning(ASOPP).MethodsEighty-two ASOPP patients in Hemodialysis Unit,Shouguang People’s Hospital were selected and divided into two groups according to the random number table method from April 2021 to April 2022.The control group was given conventional medical treatment and hemoperfusion,and the observation group was treated with sequential hemodialysis on the basis of the treatment of control group.The clinical efficacy,recovery and myocardial function of the two groups were compared.ResultsThe total effective rate of treatment in the observation group was higher than that of the control group(P<0.05).The recovery time of serum cholinestorase activity,coma time were shorter than those of the control group,and the dosage of atropine was less than that in the control group(P<0.05).The differences were not statistically significant in creatine kinase isoenzyme,troponin T,creatine kinase,and lactate dehydrogenase in the two groups before treatment.The creatine kinase isoenzyme,troponin T,creatine kinase,and lactate dehydrogenase in the observation group were lower than those in the control group after treatment(P<0.05).ConclusionSequential blood purification was effective in treating patients with ASOPP,which can protect myocardial function,reduce myocardial injury,and accelerate symptom relief.
