摘　　要：目的观察新型冠状病毒感染(COVID-19)患者外周血淋巴细胞亚群计数及百分比的临床特征。方法回顾性分析台州市中医院收治的77例COVID-19患者的临床资料,根据诊疗方案对纳入的病例进行分组,对各组患者外周血淋巴细胞亚群进行分析比较,并使用Pearson相关性分析评估血氧饱和度水平与CD4/CD8比值之间的关系。结果77例确诊患者中包括轻型26例,中型39例,重型7例,危重型5例;相比于轻型及中型,重型及危重型患者外周血CD3+T细胞、CD4+T细胞、CD8+T细胞、B细胞和NK细胞计数均明显降低;而CD8+T细胞百分比显著升高。重型及危重型组患者外周血CD4/CD8比值高于中型及轻型患者;COVID-19患者血氧饱和度水平与CD4/CD8比值具有负相关性。结论COVID-19患者,尤其是重型及危重型患者外周血淋巴细胞计数显著下降,CD4/CD8比值可能是评估COVID-19患者病情轻重程度的有效指标。Objective To observe the clinical characteristics of peripheral blood lymphocyte subset counts and percentages in patients with novel coronavirus infection(COVID-19).Methods The basic data of 77 patients with COVID-19 admitted to Taizhou City Hospital of Traditional Chinese Medicine were retrospectively analyzed,the included cases were grouped by treatment protocol,the peripheral blood lymphocyte subsets of each group were analyzed and compared,and the relationship between blood oxygen saturation level and CD4/CD8 ratio was assessed using Pearson correlation analysis.Results The 77 patients diagnosed included 26 light,39 medium,7 heavy,and 5 critical.Compared with light and medium,peripheral blood CD3+T-cell,CD4+T-cell,CD8+T-cell,B-cell,and NK-cell counts were significantly lower in heavy and critical patients;whereas the percentage of CD8+T-cells was significantly higher.The peripheral blood CD4/CD8 ratio was higher in patients in the severe and critical groups than in medium and light patients;there was a negative correlation between blood oxygen saturation levels and CD4/CD8 ratio in patients with COVID-19 infection.Conclusion Peripheral blood lymphocyte counts were significantly decreased in patients with COVID-19,especially in the heavy and critical groups,and the CD4/CD8 ratio may be a valid indicator to assess the severity of the disease in patients with COVID-19.
