改良振幅整合脑电图评分在重度高胆红素血症新生儿神经预后中的应用价值

摘　　要：目的探究改良振幅整合脑电图(aEEG)评分在重度高胆红素血症新生儿神经预后中的应用价值。方法选取2020年1月至2021年12月上饶市人民医院收治的80例重度高胆红素血症新生儿作为研究对象,按照住院期间aEEG评分分为正常组(n=23)、轻度异常组(n=30)与重度异常组(n=27)。比较3组体格发育、智力发育情况。结果3组体质量、头围、身长的时间、组间、交互效应比较差异有统计学意义(P<0.05);6、12月龄,重度异常组体质量轻于正常组、轻度异常组,头围小于正常组、轻度异常组,身长短于正常组、轻度异常组,差异有统计学意义(P<0.05),正常组与轻度异常组体质量、头围、身长比较差异无统计学意义。3组智力发育指数(MDI)评分组间、时间比较差异有统计学意义(P<0.05),交互效应比较差异无统计学意义;3组应人能、应物能、言语能及个体动作能评分的时间、组间、交互效应比较差异有统计学意义(P<0.05)。6、12月龄,重度异常组MDI评分及应人能、应物能、言语能、个体动作能评分均低于正常组、轻度异常组,轻度异常组低于正常组,差异有统计学意义(P<0.05)。结论改良aEEG评分异常程度可作为重度高胆红素血症患儿神经预后的评估指标,值得临床推广应用。Objective To investigate the application value of modified amplitude-integrated electroencephalogram(aEEG)score in neurological prognosis of neonates with severe hyperbilirubinemia.Methods 80 neonates with severe hyperbilirubinemia admitted to Shangrao People's Hospital from January 2020 to December 2021 were selected as the research subjects,they were divided into the normal group(n=23),the mild abnormality group(n=30)and the severe abnormality group(n=27)according to the aEEG score during hospitalization.Physical and intelligence development were compared among three groups.Results There were significant differences in weight,head circumferences and height among the three groups of group,time piont and interaction(P<0.05);at 6 and 12 months old,the weight in the severe abnormality group was slighter than that in the nor-mal group and the mild abnormality group,head circumferences was smaller than that in the normal group and the mild abnormality group,height was shorter than that in the normal group and the mild abnormality group,and the differences were satistically significant(P<0.05),there were no significant differences in weight,head circumferences and height between the normal group and the mild abnormality group.There were significant differences in mental development index(MDI)scores among three groups of group and time piont(P<0.05),there was no significant difference in interaction effects among three groups;there were significant differences in personalsocial behavior,coping with material ability,language behavior and individual action ability scores among the three groups of group,time piont and interaction(P<0.05).At 6 and 12 months old,the scores of MDI,personalsocial behavior,coping with material ability,language behavior and individual action ability in the severe abnormality group were lower than those in the normal group and the mild abnormality group,and the mild abnormality group was lower than the normal group,and the differences were satistically significant(P<0.05).Conclusion The abnormal
