摘 要:目的探讨采用阴道多普勒超声检查对肥胖型与非肥胖型多囊卵巢综合征(PCOS)患者卵巢血流参数与激素水平的相关性。方法选取96例PCOS患者,根据体质量指数(BMI)分为肥胖组(42例,≥25 kg/m^(2))、非肥胖组(54例,<25 kg/m^(2))。两组均接受阴道多普勒超声检查,测定卵巢血流参数,并测定激素水平,分析卵巢血流参数与激素水平的关系。结果肥胖组搏动指数(PI)、阻力指数(RI)较非肥胖组高(P<0.05);两组收缩期最大血流速度(PSV)、舒张期最大血流速度(EDV)对比,差异无统计学意义(P>0.05);肥胖组促黄体生成素(LH)、卵泡刺激素(FSH)水平较非肥胖组低(P<0.05);两组孕酮(P)、睾酮(T)、催乳素(PRL)、雌二醇(E_(2))水平对比,差异无统计学意义(P>0.05)。经Pearson相关性分析检验显示,PCOS患者PI、RI与LH、FSH水平成负相关(P<0.05)。结论肥胖型与非肥胖型PCOS患者阴道多普勒超声检查卵巢血流参数有差异,肥胖型患者以灌流量增加为主,且与LH、FSH激素水平有关。Objective To investigate the correlation between ovarian blood flow parameters and hormone levels in patients with obese and nonobese polycystic ovary syndrome(PCOS).Methods 96 PCOS patients were selected and divided into obese group(42 cases,≥25 kg/m^(2))and non-obese group(54 cases,<25 kg/m^(2))according to body mass index(BMI).Both groups underwent vaginal Doppler ultrasonography to determine ovarian blood flow parameters and hormone levels,and analyze the relationship between ovarian blood flow parameters and hormone levels.Results The pulsatility index(PI)and resistance index(RI)of the obese group were higher than those of the non-obese group(P<0.05).There was no significant difference in systolic maximum blood flow velocity(PSV)and diastolic maximum blood flow velocity(EDV)between the two groups(P>0.05).The levels of luteinizing hormone(LH)and follicle stimulating hormone(FSH)in obese group were lower than those in nonobese group(P<0.05).There was no significant difference in the levels of progesterone(P),testosterone(T),prolactin(PRL)and estradiol(E_(2))between the two groups(P>0.05).The Pearson correlation analysis showed that PI and RI were negatively correlated with LH and FSH levels in PCOS patients(P<0.05).Conclusion The ovarian blood flow parameters were different in obese and non-obese PCOS patients,which was mainly related to LH and FSH hormone levels.
