基于络病理论探讨慢性萎缩性胃炎的病程演变及防治

摘　　要：慢性萎缩性胃炎(chronic atrophic gastritis,CAG)是一种多发性、慢性、具有恶变可能的消化系统疾病。因CAG的发病机制复杂且尚未完全阐明,其治疗方法存在一定的局限性,因此寻求阻止甚至逆转CAG的方法一直是国内外关注的热点及难点。西医在CAG的治疗跟逆转方面效果不佳。而CAG的发展过程及病理特点,与络病理论的“久病入络、易滞易瘀、易入难出”的特点不谋而合。因此文章从络病理论出发,以CAG现有的治疗体系及自然病程为依据,根据络病理论“郁-瘀-虚-损”的病机转变特点将其分为初始-发病-转变-终末四期论治。并按照正邪虚实转变及相关病因,给予对应治疗措施。以求为CAG的防治提供新的思考方向。并为络病理论应用于其他疾病的诊治提供新的探讨方法。Chronic atrophic gastritis(CAG)is a multiple,chronic and potentially malignant digestive disease.Because the pathogenesis of CAG is complex and has not been fully elucidated,and its treatment methods have certain limitations,seeking ways to prevent or even reverse CAG has been a hot spot and difficulty at home and abroad.Western medicine is not effective in the treatment and reversal of CAG.The development process and pathological characteristics of CAG coincide with the characteristics of“chronic disease entering collaterals,easy stasis,easy entry and difficult exit”in the collaterals theory.Therefore,this paper is based on the theory of collateral disease and the existing treatment system and natural course of CAG.According to the pathogenesis of the theory of collaterals disease,it can be divided into four stages:initial stage,onset stage,transition stage and end stage.Corresponding treatment measures were given according to the transformation of positive and evil deficiency and deficiency and related causes.In order to provide a new thinking direction for the prevention and treatment of CAG.It also provides a new method to explore the application of collaterals theory in the diagnosis and treatment of other diseases.
