摘　　要：糖尿病及其并发症造成了严峻的全球性公共卫生问题,糖尿病心肌病是一种以心肌细胞肥大、凋亡和纤维化进展为主要特征的特殊心肌病,是糖尿病的严重并发症之一。近年来在传统中医理论的基础上,结合现代医学研究成果,对糖尿病心肌病的中医病机探究不断延展深入。从“消渴病”“心悸”“怔忡”“胸痹”等泛化归类,到“消渴心病”“消渴心瘅”的探索,糖尿病心肌病的中医病名日趋明确。基于疾病病理、病程等特点,传统气血、燥热、脏腑、瘀血病机理论被进一步应用,络病、瘀毒、瘀热、浊毒、热毒等创新性病机学说被提出,为糖尿病心肌病治疗奠定了丰富的理论基础。糖尿病心肌病病机理论的创新发展完善了糖尿病并发症理法方药体系,填补了中医现代疾病病机研究的空白。提炼与总结针对糖尿病心肌病这一现代疾病中医病机理论的认知,对于丰富现代中医理论内涵架构具有现实意义。Diabetes and its complications have caused serious global public health problems.Diabetic cardiomyopathy(DCM)is a special cardiomyopathy characterized by cardiomyocyte hypertrophy,apoptosis and fibrosis progression,and is one of the serious complications of diabetes.In recent years,on the basis of traditional Chinese medicine(TCM)theory and modern medical research results,the research on the pathogenesis of DCM in TCM has been extended and deepened.From the generalized classification of“consumptive thirst”“palpitation”“severe palpitation”“chest painful impediment”and so on,to the exploration of“consumption heart disease”and“consumption heart heat disease”,the TCM disease name of DCM is becoming increasingly clear.Based on the characteristics of disease pathology and course,the TCM pathogenesis theories of Qi-blood,dryness-heat,Zang-Fu organs and Blood stasis have been further applied,and a wealth of innovative pathogenesis theories such as collateral disease,blood-stasis toxin,blood-stasis heat,turbid toxic and heat-toxicity have been proposed,laying a rich theoretical foundation for the treatment of DCM.The innovative development of the pathogenesis theory of DCM has improved the theory,prescription and medicine system of diabetic complications and filled the gap of modern disease pathogenesis research in TCM.It is of practical significance to enrich the theoretical connotation and framework of modern TCM to refine and summarize the theory of pathogenesis of DCM.
