双侧切口双侧钢板内固定术治疗胫骨平台骨折患者的效果

摘　　要：目的:观察双侧切口双侧钢板内固定术治疗胫骨平台骨折患者的效果。方法:回顾性分析2020年1月至2023年1月该院收治的60例胫骨平台骨折患者的临床资料,按照手术方法不同将其分为对照组和观察组各30例。对照组采用单侧切口单侧钢板内固定术治疗,观察组采用双侧切口双侧钢板内固定术治疗,比较两组手术相关指标(术中出血量、手术时间、住院时间和骨折愈合时间)水平、膝关节功能恢复优良率、手术前后疼痛[视觉模拟评分法(VAS)]和膝关节功能[美国特种外科医院膝关节功能量表(HSS)]评分,以及并发症发生率。结果:两组术中出血量、手术时间比较,差异均无统计学意义(P>0.05);观察组住院时间、骨折愈合时间均短于对照组,差异有统计学意义(P<0.05);观察组膝关节功能恢复优良率为96.67%(29/30),高于对照组的73.33%(22/30),差异有统计学意义(P<0.05);术后1、3个月,观察组VAS评分均低于对照组,HSS评分均高于对照组,差异有统计学意义(P<0.05);两组并发症发生率比较,差异无统计学意义(P>0.05)。结论:双侧切口双侧钢板内固定术治疗胫骨平台骨折患者可缩短住院时间和骨折愈合时间,提高膝关节功能恢复优良率和膝关节功能评分,降低疼痛评分,效果优于单侧切口单侧钢板内固定术治疗。Objective:To observe effects of bilateral incision bilateral plate internal fixation in treatment of patients with tibial plateau fractures.Methods:The clinical data of 60 patients with tibial plateau fractures admitted to this hospital from January 2020 to January 2023 were retrospectively analyzed.According to different surgical methods,they were divided into control group and observation group,30 cases in each group.The control group was treated with unilateral incision unilateral plate internal fixation,while the observation group was treated with bilateral incision bilateral plate internal fixation.The operation related indexes(intraoperative blood loss,operation time,hospitalization time and fracture healing time),the excellent and good rate of knee function recovery,the pain[visual analogue scale(VAS)]and the knee joint function[hospital for special surgery knee function scale(HSS)]scores before and after the surgery,and the incidence of complications were compared between the two groups.Results:There were no significant differences in the intraoperative blood loss and the operation time between the two groups(P>0.05).The hospitalization time and the fracture healing time of the observation group were shorter than those of the control group,and the differences were statistically significant(P<0.05).The excellent and good rate of knee function recovery in the observation group was 96.67%(29/30),which was higher than 73.33%(22/30)in the control group,and the difference was statistically significant(P<0.05).1 and 3 months after the surgery,the VAS scores of the observation group were lower than those of the control group,the HSS scores were higher than those of the control group,and the differences were statistically significant(P<0.05).However,there was no significant difference in the incidence of complications between the two groups(P>0.05).Conclusions:Bilateral incision bilateral plate internal fixation in the treatment of patients with tibial plateau fracture can shorten the hospitalization time and the
