六郁汤治疗非典型抗精神病药物所致代谢综合征效果及安全性的Meta分析

摘　　要：目的基于Meta分析评价六郁汤治疗非典型抗精神病药物所致代谢综合征(MS)效果及安全性。方法计算机检索中国知网、维普网、万方数据知识服务平台、PubMed、Embase、Cochrane Library,查找公开发表六郁汤治疗非典型抗精神病药物所致MS的文献,检索时间为建库至2023年6月。在非典型抗精神病药物的基础上,研究组采用六郁汤,对照组采用安慰剂或未采取干预措施,采用RevMan 5.4软件进行Meta分析,绘制漏斗图评估文献发表偏倚风险。结果最终纳入6篇文献,共包含668例患者,其中研究组359例,对照组309例。Meta分析结果显示,研究组体质指数(BMI)[WMD=-1.44,95%CI(-1.91,-0.97),P<0.001]、空腹血糖(FBG)[WMD=-1.15,95%CI(-1.76,-0.53),P<0.001]、三酰甘油(TG)[WMD=-0.28,95%CI(-0.42,-0.15),P<0.001]、治疗后副反应量表(TESS)评分[WMD=-0.81,95%CI(-1.39,-0.24),P<0.001]、阳性和阴性症状量表(PANSS)总分[WMD=-7.96,95%CI(-12.18,-3.73),P<0.001]均低于对照组,1篇文献研究表明研究组患者的总胆固醇较治疗前明显降低且具有统计学意义,另1篇文献结果表明治疗前后研究组与对照组间TC无显著差异。绘制漏斗图结果显示,报道BMI的文献存在发表偏倚,考虑可能与纳入文献较少相关;报道FBG、TG、TESS评分、PANSS总分的文献散点左右分布对称,提示不存在发表偏倚。结论六郁汤治疗非典型抗精神病药物所致MS具有良好的疗效,可有效改善患者糖脂代谢水平,且不良反应少,安全性更高。Objective To evaluate the efficacy and safety of Liuyu decoction in the treatment of MS induced by atypical antipsychotics based on Meta-analysis.Methods CNKI,VIP,Wanfang Data,PubMed,Embase,and Cochrane Library were searched for published literature on the treatment of metabolic syndrome caused by atypical antipsychotic drugs from the establishment of the database to June 2023.On the basis of atypical antipsychotics,the study group used Liuyu decoction,the control group received placebo or no intervention,and RevMan 5.4 software was used for Meta-analysis,and a funnel plot was drawn to assess the risk of publication bias.Results Six studies were included,including 668 patients,a total of 359 cases in the study group and 309 cases in the control group.The results of Meta-analysis showed that the BMI[WMD=-1.44,95%CI(-1.91,-0.97),P<0.001],FBG[WMD=-1.15,95%CI(-1.76,-0.53),P<0.001],TG[WMD=-0.28,95%CI(-0.42,-0.15),P<0.001],TESS score[WMD=-0.81,95%CI(-1.39,-0.24),P<0.001],and PANSS total score[WMD=-7.96,95%CI(-12.18,-3.73),P<0.001]were lower than those of the control group,one literature showed that the total cholesterol of the study group was significantly lower than that before treatment,and the other literature showed that there was no significant difference in TC between the study group and the control group before and after treatment.The results of funnel plot showed that the literature reporting BMI had publication bias,which may be related to the lack of included literature.The scattered points of the literature reporting FBG,TG,TESS score,and PANSS total score were symmetrical,indicating that there was no publication bias.Conclusion Liuyu decoction has a good effect in the treatment of MS caused by atypical antipsychotic drugs,and can effectively improve the level of glucose and lipid metabolism in patients,with fewer adverse reactions and higher safety.
