新伤续断汤加减联合冷热敷及按摩治疗四肢骨折后急性疼痛肿胀的效果

摘　　要：目的观察新伤续断汤加减联合冷热敷及按摩治疗四肢骨折后急性疼痛肿胀的效果。方法选取2021年1月—2022年12月泉州市中医院收治的四肢骨折后急性疼痛肿胀患者60例,病例分组方法为随机数字表法,常规西医治疗组30例采用常规西医治疗,新伤续断汤组30例在常规西医治疗组基础上采用新伤续断汤加减联合冷热敷及按摩治疗,2组均治疗30 d。比较2组治疗前后中医证候积分、视觉模拟评分法(VAS)评分、肿胀程度及骨折愈合指标[骨形态发生蛋白(BMP)、Ⅰ型前胶原羧基端肽(PⅠCP)、胰岛素生长因子(IGF)、骨碱性磷酸酶(ALP)]。结果治疗30 d后,2组各项中医证候积分均较治疗前降低,且与常规西医治疗组各项中医证候积分相比,新伤续断汤组明显更低(P<0.01);2组VAS评分及肿胀程度均较治疗前降低,且与常规西医治疗组上述指标相比,新伤续断汤组明显更低(P<0.01);2组BMP、PⅠCP、IGF及ALP水平均较治疗前升高,且与常规西医治疗组上述指标相比,新伤续断汤组明显更高(P<0.01)。结论四肢骨折后急性疼痛肿胀患者采用新伤续断汤加减联合冷热敷及按摩治疗的效果较好,可有效改善中医证候,减轻疼痛和肿胀程度,促进骨折愈合。Objective To observe the effect of Xinshang Xuduan decoction combined with cold and hot compress and massage on acute pain and swelling after fracture of limbs.Methods Sixty patients with acute pain and swelling after limb fracture treated in Quanzhou Hospital of Traditional Chinese Medicine from January 2021 to December 2022 were selected.The case grouping method was random number table method.30 cases in the conventional western medicine treatment group were treated with conventional western medicine,and 30 cases in the Xinshang Xuduan decoction group were treated with Xinshang Xuduan decoction combined with cold and hot compress and massage on the basis of the conventional western medicine treatment group.Both groups were treated for 30 days.TCM syndrome score,visual analogue scale(VAS)score,swelling degree and fracture healing indexes(BMP,PⅠCP,IGF,ALP)before and after treatment were compared between the 2 groups.Results After 30 days of treatment,the scores of TCM syndroms in the 2 groups were lower than those before treatment,and the scores of TCM syndroms in the Xinshang Xuduan decoction group were significantly lower than those in the conventional western medicine treatment group(P<0.01).VAS score and swelling degree in 2 groups were lower than before treatment,and compared with conventional western medicine treatment group,the above indexes in Xinshang Xuduan decoction group were significantly lower(P<0.01).The levels of BMP,PⅠCP,IGF and ALP in the 2 groups were higher than those before treatment,and those in the Xinshang Xuduan decoction group were significantly higher than those in the conventional western medicine treatment group(P<0.01).Conclusion Patients with acute pain and swelling after fracture of extremities were treated with Xinshang Xuduan decoction combined with cold and hot compress and massage,which could effectively improve TCM syndrome,reduce pain and swelling,and promote fracture healing.
