麝香保心丸联合阿司匹林肠溶片对冠心病患者颈动脉斑块、NT-proBNP、肌钙蛋白I的影响

摘　　要：目的观察麝香保心丸联合阿司匹林肠溶片对冠心病患者颈动脉斑块、N末端脑钠肽前体(NT-proBNP)、肌钙蛋白I(cTnI)的影响。方法选取2020年7月—2023年4月武夷山市立医院收治的冠心病患者80例,以随机数字表法分为中西联合组和阿司匹林组,各40例。2组均行常规对症治疗,在此基础上,中西联合组予麝香保心丸联合阿司匹林肠溶片治疗,阿司匹林组予阿司匹林肠溶片治疗,2组均治疗12周。比较2组疗效,治疗前后颈动脉斑块厚度、内膜—中层厚度(IMT)、血脂指标[三酰甘油(TG)、总胆固醇(TC)、高密度脂蛋白胆固醇(HDL-C)、低密度脂蛋白胆固醇(LDL-C)]、NT-proBNP、cTnI水平及不良反应。结果中西联合组总有效率为92.50%,高于阿司匹林组的75.00%(χ^(2)=4.501,P=0.034)。治疗12周后,2组颈动脉斑块厚度、IMT、TG、TC、LDL-C、NT-proBNP、cTnI水平均低于治疗前,HDL-C水平高于治疗前,且中西联合组低/高于阿司匹林组(P均<0.01)。治疗期间,2组未见明显不良反应。结论麝香保心丸联合阿司匹林治疗冠心病疗效良好,可显著减少患者颈动脉斑块厚度、IMT,调节血脂,改善NT-proBNP、cTnI表达水平,且安全性高。Objective To observe the effects of Shexiang Baoxin pills combined with aspirin enteric-coated tablets on carotid plaque,N-terminal pro brain natriuretic peptide(NT-proBNP)and troponin I(cTnI)in patients with coronary heart disease.Methods A total of 80 patients with coronary heart disease admitted to Wuyishan City Hospital from July 2020 to April 2023 were selected and randomly divided into the combined Chinese and Western medicine group and the aspirin group,40 cases in each group.Both groups were treated with conventional symptomatic treatment.On this basis,the combined Chinese and Western medicine group was treated with Shexiang Baoxin pills combined with aspirin enteric-coated tablets,and the aspirin group was treated with aspirin enteric-coated tablets.Both groups were treated for 12 weeks.The curative effects,carotid plaque thickness,IMT,blood lipid indexes(TG,TC,HDL-C,LDL-C),NT-proBNP,cTnI levels before and after treatment,and adverse reactions were compared between the two groups.Results The total effective rate of the combined Chinese and Western medicine group was 92.50%,which was higher than 75.00%of the aspirin group(χ^(2)=4.501,P=0.034).After 12 weeks of treatment,the carotid plaque thickness,IMT,TG,TC,LDL-C,NT-proBNP and cTnI levels in the two groups were lower than those before treatment,while the HDL-C level was higher than that before treatment,and the combined Chinese and Western medicine group was lower/higher than that in the aspirin group(P<0.01).During the treatment,no obvious adverse reactions were observed in the two groups.Conclusion Shexiang Baoxin pills combined with aspirin has good curative effect in the treatment of coronary heart disease,which can significantly reduce the thickness of carotid plaque and IMT,regulate blood lipids,improve the expression levels of NT-proBNP and cTnI,and has high safety.
