“热补法”针法配合补阳还五汤对脑卒中恢复期肌张力增高患者运动功能及生活能力的影响研究

摘　　要：目的:探究“热补法”针法配合补阳还五汤对脑卒中恢复期肌张力增高患者运动功能及生活能力的影响。方法:将2022年1月—2023年6月酒泉市中医医院的80例脑卒中恢复期肌张力增高患者根据随机数字表法分为两组,每组40例。对照组按照常规针刺配合补阳还五汤方案治疗,观察组则按照“热补法”针法配合补阳还五汤方案治疗。比较两组的肌张力增高治疗总有效率、治疗前后的肌张力情况(改良Ashworth分级)、运动功能[伯格平衡量表(BBS评分)、Holden步行能力分级及步态参数(步速及步幅)]及生活能力(改良Barthel指数)。结果:观察组的肌张力增高治疗总有效率显著高于对照组,差异有统计学意义(P<0.05)。治疗前两组观察指标的差异均无统计学意义(P>0.05),具体包括改良Ashworth分级、BBS评分、Holden步行能力分级、步态参数及改良Barthel指数;治疗3个疗程后观察组的改良Ashworth分级及Holden步行能力分级构成均显著优于对照组,BBS评分、步态参数及改良Barthel指数均显著高于对照组,差异均有统计学意义(P<0.05)。结论:在脑卒中恢复期肌张力增高患者的治疗中,“热补法”针法配合补阳还五汤这一方法临床效果较好,优势明显,可显著改善患者的运动功能及生活能力,应用价值较高。Objective:To investigate the influence of heat-reinforcing needling manipulation combined with Buyang Huanwu Decoction for motor function and living ability of patients with hypermyotonia during the recovery period of stroke.Method:A total of 80 patients with hypermyotonia during the recovery period of stroke in Traditional Chinese Medicine Hospital of Jiuquan City from January 2022 to June 2023 were divided two groups according to the random number table.40 cases of control group were treated with routine acupuncture therapy combined with Buyang Huanwu Decoction,40 cases of observation group were treated with heat-reinforcing needling manipulation combined with Buyang Huanwu Decoction.Then the total effective rates for hypermyotonia,the muscular tension situation(modified Ashworth classification),motor function[Berg balance scale(BBS score),Holden walking ability grades and gait parameters(step speed and stride)]and living ability(modified Barthel index)before and after the treatment of two groups were compared.Result:The total effective rate for hypermyotonia of observation group was significantly higher than that of control group,the difference was statistically significant(P<0.05),the differences of modified Ashworth classification,BBS score,Holden walking ability grades,gait parameters and modified Barthel index of two groups before the treatment were not statistically significant(P>0.05),the modified Ashworth classification and Holden walking ability grades of observation group at 3rd course of treatment were significantly better than those of control group,the BBS score and gait parameters and modified Barthel index were significantly higher than those of control group,the differences were statistically significant(P<0.05).Conclusion:The clinical effect of heat-reinforcing needling manipulation combined with Buyang Huanwu Decoction in patients with hypermyotonia during the recovery period of stroke is better,and it can significantly improve the motor function and living ability of patients,so its applicati
