集束化护理在奥沙利铂化疗致周围神经病变中的应用研究

摘　　要：目的:分析集束化护理在奥沙利铂化疗致周围神经病变中的应用效果。方法:选择2020年7月—2023年6月在赣南医学院第一附属医院规范接受奥沙利铂化疗致周围神经病变的60例患者作为研究对象,按照随机数字表法分为两组,各30例。对照组采用常规护理,观察组采用集束化护理,对比两组的护理效果、平衡功能、心理状态及生活质量。结果:护理1个月后,两组化疗所致周围神经病变评估工具(chemotherapy-induced peripheral neuropathy assessment tool,CIPNAT)各维度评分及总分均低于护理前,且观察组均低于对照组,差异均有统计学意义(P<0.05)。护理1个月后,两组Berg平衡量表(Berg balance scale,BBS)评分均高于护理前,且观察组高于对照组,差异均有统计学意义(P<0.05)。护理1个月后,两组医院焦虑抑郁情绪测量表(hospital anxiety and depression scale,HAD)评分均低于护理前,且观察组低于对照组,差异均有统计学意义(P<0.05)。护理1个月后,两组世界卫生组织生活质量-100量表(WHO quality of life-100,WHOQOL-100)评分均高于护理前,且观察组高于对照组,差异均有统计学意义(P<0.05)。结论:集束化护理能够有效减轻奥沙利铂化疗致周围神经病变严重程度,减轻疾病对患者日常生活的影响,保证平衡功能,从而改善心理状态和生活质量。Objective:To analyze the effect of cluster nursing in the treatment of peripheral neuropathy caused by Oxaliplatin chemotherapy.Method:A total of 60 patients with peripheral neuropathy caused by Oxaliplatin chemotherapy in the First Affiliated Hospital of Gannan Medical College from July 2020 to June 2023 were divided into two groups according to random number table method,with 30 patients in each group.The control group was treated with routine nursing,and the observation group was treated with cluster nursing.The nursing effect,balance function,psychological state and quality of life of two groups were compared.Result:After 1 month of nursing,the scores of all dimensions and total scores of chemotherapy-induced peripheral neuropathy assessment tool(CIPNAT)in two groups were lower than those before nursing,and those in observation group were lower than those in control group,the differences were statistically significant(P<0.05).After 1 month of nursing,the scores of Berg balance scale(BBS)in two groups were higher than those before nursing,and that in the observation group was higher than that in the control group,the differences were statistically significant(P<0.05).After 1 month of nursing,the scores of hospital anxiety and depression scale(HAD)of the two groups were lower than those before nursing,and that in the observation group was lower than that in the control group,the differences were statistically significant(P<0.05).After 1 month of nursing,the WHO quality of life-100(WHOQOL-100)scores of two groups were higher than those before nursing,and that in the observation group was higher than that in the control group,the differences were statistically significant(P<0.05).Conclusion:Cluster nursing can effectively reduce the degree of peripheral neuropathy caused by Oxaliplatin chemotherapy,reduce the impact of the disease on patients'daily life,ensure the balance function,and thus improve the psychological state and quality of life.
