摘 要:目的 研究维生素D缺乏对儿童哮喘发病及哮喘控制的影响。方法 选取2020年1月至2022年12月陕西省渭南市第一医院收治的141例哮喘患儿作为试验组,另选取同期健康体检儿童140例作为对照组。比较两组研究对象的血清维生素D水平;比较不同哮喘程度患儿的血清维生素D水平;分析哮喘控制水平的影响因素。结果 试验组的25-羟维生素D[25-(OH)D]水平低于对照组,维生素D缺乏占比高于对照组(P<0.05)。重度哮喘患儿的维生素D缺乏占比高于轻度、中度患儿,且中度哮喘患儿的维生素D缺乏占比高于轻度患儿(P<0.05)。哮喘控制良好组的年龄<6岁、既往1年哮喘发作次数>2次、病程≥2年、维生素D缺乏及污尘接触史占比均低于控制不良组(P<0.05)。多因素Logistic回归分析结果显示,年龄<6岁、既往1年哮喘发作次数>2次、病程≥2年、维生素D缺乏及污尘接触史均是哮喘控制不良的危险因素(P<0.05)。结论 维生素D缺乏与儿童哮喘发病及哮喘控制均密切相关,应予以重点关注。Objective To study the effects of vitamin D deficiency on the onset and control of asthma in children.Methods A total of 141 children with asthma admitted in the First Hospital in Weinan from January 2020 to December 2022 were selected as experimental group,and 140 children who underwent physical examination during the same period were selected as control group.The serum vitamin D levels of the two groups were compared;the levels of serum vitamin D in children with different degrees of asthma were compared;the influencing factors of asthma control level were analyzed.Results The 25-hydroxyvitamin D[25-(OH)D]level of the experimental group was lower than that of the control group,and the proportion of vitamin D deficiency was higher than that of the control group(P<0.05).The proportion of vitamin D deficiency in children with severe asthma was higher than that in children with mild and moderate asthma,and the proportion of vitamin D deficiency in children with moderate asthma was higher than that in children with mild asthma(P<0.05).The proportions of age<6 years old,number of asthma attacks in the past 1 year>2 times,course of disease≥2 years,vitamin D deficiency and dust exposure history in the good asthma control group were lower than those in the poor control group(P<0.05).Multivariate Logistic regression analysis results showed that age<6 years old,number of asthma attacks in the past year>2 times,course of disease≥2 years,vitamin D deficiency and dust exposure history were risk factors for poor asthma control(P<0.05).Conclusion Vitamin D deficiency is closely related to the onset and control of asthma in children,which should be given special attention.
