掌侧锁定钢板术治疗桡骨远端骨折的效果及对腕关节复位情况的影响

摘　　要：目的探讨掌侧锁定钢板术治疗桡骨远端骨折的效果及对腕关节复位情况的影响。方法选取2018年6月至2022年6月我院收治的80例桡骨远端骨折患者为研究对象,以随机数字法将其分为对照组和观察组,各40例。对照组采用常规手法复位石膏外固定治疗,观察组采用掌侧锁定钢板术治疗。比较两组的治疗效果。结果观察组的治疗优良率高于对照组(P<0.05)。术后6个月,观察组的尺偏角、掌倾角大于对照组,桡骨高度高于对照组(P<0.05)。术后6个月,观察组的白细胞介素-6(IL-6)、白细胞介素-8(IL-8)及C反应蛋白(CRP)水平均低于对照组(P<0.05)。两组的并发症总发生率比较,差异无统计学意义(P>0.05)。术后6个月,观察组的生活质量综合评定问卷(GQOL-74)各维度评分高于对照组(P<0.05)。结论掌侧锁定钢板术治疗桡骨远端骨折能提高治疗效果,减轻炎症反应,改善腕关节复位,促进患者生活质量提高,值得推广。Objective To investigate the effect of volar locking plate in the treatment of distal radius fracture and its influence on wrist joint reduction.Methods A total of 80 patients with distal radius fracture admitted in our hospital from June 2018 to June 2022 were selected as the research objects.The patients were divided into control group and observation group by random number method,with 40 cases in each group.The control group was treated with conventional manipulative reduction and plaster external fixation,and the observation group was treated with volar locking plate.The therapeutic effects of the two groups were compared.Results The excellent and good rate of treatment in the observation group was higher than that in the control group(P<0.05).At 6 months after operation,the ulnar deviation angle and palmar inclination angle of the observation group were larger than those of the control group,and the radius height was higher than that of the control group(P<0.05).At 6 months after operation,the levels of interleukin-6(IL-6),interleukin-8(IL-8)and C-reactive protein(CRP)in the observation group were lower than those in the control group(P<0.05).There was no significant difference in the total incidence of complications between the two groups(P>0.05).At 6 months after operation,the scores of each dimension of Generic Quality of Life Inventory-74(GQOL-74)in the observation group were higher than those in the control group(P<0.05).Conclusion The volar locking plate in the treatment of distal radius fracture can improve the therapeutic effect,reduce the inflammatory response,ameliorate the wrist joint reduction,and promote the quality of life of patients,which is worth promoting.
