摘　　要：目的探讨前馈控制护理模式联合代币制健康教育对心脏手术患儿依从性及不良情绪的影响。方法选取2021年5月—2023年9月上海交通大学医学院附属新华医院收治的225例心脏手术患儿进行研究。通过简单抽样法随机分为联合组(n=78)、代币组(n=72)、对照组(n=75)。对照组予以一般护理及健康宣教,代币组予以一般护理及代币制健康教育,联合组予以前馈控制护理模式联合代币制健康教育。3组均干预至出院,随访6个月。比较3组患儿治疗依从性、负性情绪、认知水平、不良事件、生活质量。结果干预后,联合组Morisky用药依从性问卷评分高于代币组(P=0.014)、对照组(P<0.001),儿童抑郁量表(CDI)评分均低于代币组(P=0.028)、对照组(P<0.001);联合组医院自制心脏病知晓情况问卷总分高于代币组、对照组(P<0.001)。三组不良事件发生率差异无统计学意义(P>0.05)。联合组患儿生活质量测定量表评分高于代币组、对照组(P<0.001)。结论前馈控制护理模式联合代币制健康教育可显著提高心脏手术患儿依从性,减轻抑郁情绪,提升对疾病的认知,并提升生活质量。Objective To investigate the effect of feedforward control nursing model combined with token-based health education on compliance and negative emotions in children undergoing cardiac surgery.Methods A total of 225 children who underwent cardiac surgery in Xinhua Hospital Affiliated to Shanghai Jiao Tong University School of Medicine from May 2021 to September 2023 were enrolled.They were randomly assigned to the combination group(n=78),the token group(n=72)and the control group(n=75).The control group received general nursing and health education.The token group received general nursing and token-based health education.The combination group received feedforward control nursing model combined with token-based health education.All groups were given intervention till discharge and followed up for 6 months.Comparison among the three groups was made on compliance with treatment,negative emotions,cognitive level,adverse events,and quality of life.Results After intervention,the Morisky Medication Adherence Scale score of the combination group was higher than that in the token group(P=0.014)and the control group(P<0.001),and the Children s Depression Inventory(CDI)score was lower than that in the token group(P=0.028)and the control group(P<0.001).The self-designed awareness of heart disease questionnaire total score of the combination group was higher than that in the token group and the control group(P<0.001).There was no significant difference in incidence rates of adverse events among three groups(P>0.05).The quality-of-life scale for children score of the combination group was higher than those of the token group and the control group(P<0.001).Conclusion Feedforward control nursing model combined with token-based health education can significantly improve the compliance of children undergoing cardiac surgery,alleviate depression,enhance their awareness of disease,and improve the quality of life.
