七叶皂苷钠在下肢深静脉血栓患者导管接触性溶栓术后的应用效果

摘　　要：目的:探讨七叶皂苷钠在下肢深静脉血栓(DVT)患者导管接触性溶栓术后的应用效果。方法:选取2012年1月—2018年12月成都大学附属医院收治的DVT患者60例为研究对象,根据随机数字表法分为参照组和试验组,各30例。导管接触性溶栓术后,参照组进行皮下注射低分子肝素钙治疗,试验组在参照组基础上进行七叶皂苷钠治疗。比较两组血清炎性因子水平、患肢与健侧周径差值、DVT复发率及平均严重程度评分。结果:治疗后,两组白细胞介素-6、超敏C反应蛋白、肿瘤坏死因子-α水平低于治疗前,试验组低于参照组,差异有统计学意义(P<0.05);治疗后,两组膝上20 cm、膝下15 cm患侧与健侧周径差值小于治疗前,试验组小于参照组,差异有统计学意义(P<0.05);治疗后,两组Autar量表评分低于治疗前,试验组低于参照组,差异有统计学意义(P<0.05);试验组复发率低于参照组,差异有统计学意义(P=0.020)。结论:七叶皂苷钠应用于DVT患者导管接触性溶栓术后,可以降低炎性介质水平,缓解下肢水肿,降低血栓严重程度及复发率。Objective:To investigate the effect of sodium aesculin in patients with deep vein thrombosis(DVT)after catheter contact hemolysis.Methods:Sixty patients with DVT admitted to the Affiliated Hospital of Chengdu University from January 2012 to December 2018 were selected as the study objects,and were divided into the reference group and the experimental group according to random number table method,with thirty cases in each group.After catheter contact hemolysis,the reference group was treated with subcutaneous injection of low molecular weight heparin calcium,and the experimental group was treated with sodium aesculin based on the reference group.The levels of serum inflammatory factors,the difference between the circumference of the affected limb and the healthy side,the recurrence rate of DVT and the average severity score of DVT were compared between the two groups.Results:After treatment,the levels of interleukin-6,hypersensitive C-reactive protein and tumor necrosis factor-αin two groups were lower than before treatment,and those in experimental group were lower than those in reference group,the difference was statistically significant(P<0.05);after treatment,the difference of circumferential diameter between the affected limb of 20 cm above the knee and 15 cm below the knee and the healthy side between the two groups was smaller than before treatment,and the experimental group was smaller than the reference group,the difference was statistically significant(P<0.05);after treatment,the Autar scale score of the two groups was lower than before treatment,and the experimental group was lower than the reference group,the difference was statistically significant(P<0.05);the recurrence rate of experimental group was lower than that of reference group,and the difference was statistically significant(P=0.020).Conclusion:After the application of sodium aesculin in DVT patients with catheter contact embolysis,it can reduce the level of inflammatory mediators,relieve lower limb edema,and reduce the severity and recurren
