经皮钢板接骨联合锁定加压钢板固定治疗胫骨远端骨折的效果及安全性分析

摘　　要：目的:分析经皮钢板接骨联合锁定加压钢板固定治疗胫骨远端骨折的效果及安全性。方法:选取商洛市丹凤县医院骨科2019年6月—2023年11月收治的胫骨远端骨折患者80例为研究对象,按照随机数字表法分为对照组与研究组,各40例。对照组采用切开复位钢板固定手术治疗,研究组则采用经皮钢板接骨联合锁定加压钢板固定手术治疗。对比两组临床疗效、踝关节功能、疼痛程度、术后并发症发生情况、围术期指标水平。结果:研究组治疗优良率高于对照组(P=0.023);术后1个月,两组美国足踝外科协会踝与后足功能量表评分升高,视觉模拟评分法评分降低,研究组优于对照组(P<0.05);研究组并发症总发生率低于对照组(P=0.045);研究组骨折愈合时间、住院时间短于对照组,术中出血量少于对照组(P<0.001)。结论:经皮钢板接骨联合锁定加压钢板固定手术在胫骨远端骨折患者康复治疗中的效果较好,能够促进踝关节功能恢复,减轻疼痛,降低术后并发症发生率、改善围术期指标。Objective:To analyze the efficacy and safety of percutaneous plate osteosynthesis combined with locking compression plate fixation in the treatment of distal tibial fractures.Methods:A total of eighty patients with distal tibial fracture admitted to the Department of Orthopedics of Shangluo Danfeng County Hospital from June 2019 to November 2023 were selected as the study objects,and were divided into control group and study group according to random number table method,with forty cases in each group.The control group was treated with open reduction plate fixation,while the study group was treated with percutaneous plate fixation combined with locking compression plate fixation.The clinical efficacy,ankle joint function,pain degree,postoperative complications and perioperative indexes were compared between the two groups.Results:The treatment rate of the study group was higher than that of the control group(P=0.023);one month after surgery,the scores of the American foot and ankle surgical association's ankle and hind foot function scale increased,and the scores of visual analog scale decreased,and the study group was better than the control group(P<0.05);the total complication rate of the study group was lower than that of the control group(P=0.045);the fracture healing time and hospitalization time of the study group were shorter than those of the control group,and the intraoperative blood loss was less than that of the control group(P<0.001).Conclusion:Percutaneous plate grafting combined with locking compression plate fixation has a good effect in the rehabilitation of patients with distal tibial fractures,which can promote the recovery of ankle joint function,relieve pain,reduce the incidence of postoperative complications and improve perioperative indexes.
