摘　　要：目的:分析社区随访健康教育在糖尿病合并高血压患者健康管理中的应用效果。方法:选取2020年8月—2023年8月于青岛市城阳区流亭街道卫生院卫生健康科接受社区随访的高血压合并糖尿病患者150例作为研究对象,采用随机数字表法将其分为对照组与观察组,每组75例。对照组采用常规社区随访,观察组在对照组基础上联合健康教育。比较两组血糖、血压、疾病知识掌握情况。结果:干预后,两组空腹血糖、收缩压、舒张压降低,观察组低于对照组(P<0.05);观察组疾病知识总掌握率高于对照组(P<0.001)。结论:社区随访健康教育在糖尿病合并高血压患者健康管理中的应用效果显著,可以降低患者血糖和血压,并提高其疾病知识掌握率。Objective:To analyze the application effect of community follow-up health education in the health management of patients with diabetes mellitus combined with hypertension.Methods:One hundred and fifty patients with hypertension combined with diabetes mellitus who received community follow-up in the Department of Health and Wellness of Qingdao Chengyang District Liuting Street Health Hospital from August 2020 to August 2023 were selected as the study subjects,and were divided into the control group and the observation group using the method of randomized numerical table,with seventy-five cases in each group.The control group used routine community follow-up,and the observation group combined health education on the basis of the control group.Comparison of the two groups of blood glucose,blood pressure,disease knowledge mastery.Results:After intervention,fasting blood glucose,systolic blood pressure,and diastolic blood pressure were reduced in both groups,and the observation group was lower than the control group(P<0.05);the total mastery rate of disease knowledge in the observation group was higher than that in the control group(P<0.001).Conclusion:The application of community follow-up health education in the health management of patients with diabetes mellitus combined with hypertension is effective,which can reduce patients'blood glucose and blood pressure and improve their disease knowledge mastery rate.
