育龄期男性人乳头瘤病毒感染的危险因素及基因分型分析

摘  要:目的明确育龄期男性生殖系统人乳头瘤病毒(HPV)感染的危险因素,并对不同部位感染的基因型进行分析。方法选取2022年1月至12月上海市长宁区妇幼保健院泌尿外科诊治的1205例育龄期男性作为研究对象,通过聚合酶链反应(PCR)和反向杂交相结合的方法检测样本中的HPV-DNA,比较外生殖器脱落细胞样本与精液样本HPV的检出率,采用Logistic回归分析HPV感染的危险因素。结果1205例育龄期男性在外生殖器脱落细胞样本中共检测出53例HPV阳性,阳性检出率为4.40%;在精液样本中共检测出37例HPV阳性,阳性检出率为3.07%;两种样本HPV阳性检出率比较,差异具有统计学意义(P<0.05);在低危型中,两种样本的单一感染与多重感染检出率比较,差异具有统计学意义(P<0.05);而在高危型中,两种样本的单一感染与多重感染检出率比较,差异无统计学意义(P>0.05)。首次性行为年龄(OR:2.317,95%CI:1.612~3.305)、性伴侣数量(OR:4.329,95%CI:1.326~9.491)、性伴侣是否感染HPV(OR:20.442,95%CI:4.506~48.383)、每周性生活次数(OR:3.155,95%CI:2.185~5.215)、包皮阴茎头炎(OR:18.231,95%CI:4.922~20.344)、包皮过长或包茎(OR:5.281,95%CI:1.073~9.431)均是育龄期男性HPV感染的危险因素(P<0.05)。结论育龄期男性HPV感染的危险因素主要与性行为模式和生殖健康状况密切相关,在外生殖器脱落细胞和精液中存在不同的HPV感染基因型特征。Objective To identify the risk factors of human papilloma virus(HPV)infection in the reproductive system in men of childbearing age,and to analyze the genotypes of infection in different parts.Methods A total of 1,205 men of childbearing age who were diagnosed and treated in the Department of Urology,Shanghai Changning Maternity and Infant Health Hospital from January to December 2022 were selected as the research subjects.Polymerase chain reaction(PCR)and reverse hybridization were used to detect HPV-DNA in the samples,and the detection rate of HPV in the exfoliated cell of external genitalia samples and semen samples was compared.Logistic regression was used to analyze the risk factors of HPV infection.Results A total of 53 HPV positive samples were detected in 1,205 male exfoliated cell of external genitalia samples,and the positive detection rate was 4.40%.A total of 37 semen samples were HPV positive,and the positive detection rate was 3.07%.The difference of HPV positive detection rate between the two kinds of samples was statistically significant(P<0.05).In low-risk genotype,there was a statistically significant difference in the detection rate of single infection and multiple infections between the two kinds of samples(P<0.05);but in high-risk and mixed genotype,there was no statistically significant difference in the detection rate of single infection and multiple infections between the two kinds of samples(P>0.05).Age at first sexual intercourse(OR:2.317,95%CI:1.612-3.305),number of sexual partners(OR:4.329,95%CI:1.326-9.491),whether the sexual partner was infected with HPV(OR:20.442,95%CI:4.506-48.383),frequency of sexual intercourse per week(OR:3.155,95%CI:2.185-5.215),balanitis prepuce(OR:18.231,95%CI:4.922-20.344),long prepuce or phimosis(OR:5.281,95%CI:1.073-9.431)were all risk factors for HPV infection in men of childbearing age(P<0.05).Conclusions Risk factors for HPV infection in men of childbearing age are closely related to sexual behavior patterns and reproductive health status.There are diff
