摘　　要：目的探讨血清转录因子样蛋白5(TCFL5)预测子宫内膜癌及其预后的临床价值。方法选取2016年1月至2018年6月邯郸市妇幼保健院收治的120例子宫内膜癌患者作为试验组,选取同期同院行健康体检的120名女性作为对照组。比较TCFL5在血清、子宫内膜癌组织、不同临床分期的表达水平,采用受试者工作特征(ROC)曲线的曲线下面积(AUC)分析血清TCFL5表达水平对子宫内膜癌的预测价值,采用单因素及多因素Cox回归分析血清TCFL5表达水平对子宫内膜癌的预后影响。结果试验组血清TCFL5表达水平高于对照组(P<0.001);与癌旁组织比较,TCFL5在癌组织中高表达(P<0.001);临床分期越晚,TCFL5表达水平越高(P<0.05)。TCFL5表达水平对子宫内膜癌的预测性能较高(AUC=0.907,95%CI:0.878~0.937)。单因素及多因素Cox回归分析显示,年龄和TCFL5表达水平是子宫内膜癌预后的独立影响因素(P<0.05)。结论TCFL5在子宫内膜癌患者血清中表达水平显著升高,其可作为诊断子宫内膜癌的标志物,且对患者预后具有较好的预测价值。Objective To explore the clinical value of serum transcription factor like protein 5(TCFL5)in predicting endometrial cancer and its prognosis.Methods A total of 120 patients diagnosed with endometrial cancer in Handan Maternal and Child Health Hospital from January 2016 to June 2018 were selected as the experimental group.Meanwhile,120 women who participated in health examinations in the same hospital in the same period were selected as the control group.The expression levels of TCFL5 in the serum,endometrial cancer tissues,and different clinical stages of the two groups of patients were compared.The area under the curve(AUC)of the receiver operating characteristic(ROC)curve was used to analyze the predictive value of serum TCFL5 expression levels for endometrial cancer.Univariate and multivariate Cox regression analysis were used to investigate the impact of serum TCFL5 expression levels on the prognosis of endometrial cancer.Results The expression levels of serum TCFL5 in the experimental group were significantly higher than that in the control group(P<0.001).Compared with para-carcinoma tissues,TCFL5 was highly expressed in the cancer tissues(P<0.001).The later the clinical stage,the higher the expression level of TCFL5(P<0.05).The expression level of TCFL5 had a high predictive performance for endometrial cancer(AUC=0.907,95%CI:0.878-0.937).Both univariate and multivariate Cox regression analysis confirmed that age and TCFL5 expression level were independent influencing factors for the prognosis of endometrial cancer(P<0.05).Conclusions The expression level of TCFL5 is significantly elevated in the serum of endometrial cancer patients.It can be used as a marker for the diagnosis of endometrial cancer and has good predictive value for the prognosis of patients.
