摘 要:目的观察年龄及月经状态对健康女性乳腺动态增强MRI(DCE-MRI)半定量参数的影响。方法回顾性收集283名因疑诊乳腺包块或乳腺不适而接受MR检查且经临床随访1年无乳腺肿瘤的成年女性;另前瞻性招募49名处于行经期的健康成年女性(行经期亚组)。根据年龄将上述332名受试者分为低龄组(n=107)、中龄组(n=114)及高龄组(n=111),同时根据是否绝经将其分为绝经组(n=112)与未绝经组(n=220,含行经期亚组49名、增殖期亚组77名及分泌期亚组94名)。比较组间、亚组间乳腺DCE-MRI半定量参数(ER_(max),Slope_(max)),包括最大强化率(ER_(max))和最大上升斜率(Slope_(max))的差异及其变异程度。结果高、低龄组间ER_(max)及Slope_(max)差异均有统计学意义(P均<0.05),中龄组ER_(max)和Slope_(max)与低及高龄组差异均无统计学意义(P均>0.05)。绝经组ER_(max)及Slope_(max)均低于未绝经组(P均<0.05)。不同月经周期亚组之间DCE-MRI半定量参数总体差异均无统计学意义(P均>0.05)。低、中及高龄组中,正常乳腺ER_(max)的变异系数(CV)分别为56.20%、44.02%及50.97%,Slope_(max)的CV分别为54.74%、81.78%及76.93%。绝经后及未绝经成年女性正常乳腺ER_(max)的CV分别为50.12%及46.02%,Slope_(max)的CV分别为72.84%及62.04%。各亚组间,ER_(max)及Slope_(max)在增殖期亚组的CV均最大(61.39%、82.54%),而在行月经期亚组的CV均最小(33.99%、42.33%)。结论不同年龄及月经状态下健康女性乳腺DCE-MRI半定量参数存在差异,且个体间变异度较大。Objective To observe the impact of age and menstrual status on semi-quantitative parameters of breast dynamic enhanced MRI(DCE-MRI)in healthy adult women.Methods A total of 283 adult females who underwent MR examinations due to suspected breast mass or breast discomfort but no breast tumor was detected after 1 year's clinical follow-up were retrospectively collected.Meanwhile,49 healthy adult female subjects in the menstrual period(menstruating subgroup)were prospectively recruited.All the above 332 subjects were divided into low age group(n=107),middle age group(n=114)and high age group(n=111)according to age,while into postmenopausal group(n=112)and premenopausal group(n=220,including 49 in menstruating subgroup,77 in proliferating subgroup and 94 in secreting subgroup).DCE-MRI semi-quantitative parameters,including maximum enhancement rate(ER_(max))and maximum slope of increasing(Slope_(max))were compared among different groups and subgroups,and the variations were observed.Results Significant differences of ER_(max)and Slope_(max)were found between high and low age groups(both P<0.05),while no significant difference of ER_(max)and Slope_(max)was found between middle and low age group,nor between middle and high age group(all P>0.05).Both ER_(max)and Slope_(max)in postmenopausal group were lower than those in premenopausal group(both P<0.05),while no significant difference of DCE-MRI semi-quantitative parameters was found among different menstrual cycle subgroups(all P>0.05).The coefficient of variance(CV)of normal breast ER_(max)in low,middle and high age groups was 56.20%,44.02%and 50.97%,respectively,of Slope_(max)was 54.74%,81.78%and 76.93%,respectively.CV of normal breast ER_(max)was 50.12%and 46.02%in postmenopausal and premenopausal groups,respectively,while CV of Slope_(max)was 72.84%and 62.04%,respectively.Among different subgroups,CV of ER_(max)and Slope_(max)in proliferative period were both the largest(61.39%,82.54%),which in menstrual period were both the smallest(33.99%,42.33%).Conclusion Semi-qu
