摘 要:目的 分析信息化多元运动对血液透析患者运动能力及负性情绪的作用。方法 收集2020年6月-2022年6月在郑州市中心医院进行血液透析(HD)的患者120例,随机分为对照组和观察组,各60例。对照组采用常规临床干预,观察组采用信息化多元运动干预。比较干预前后两组患者的心肺耐力、步速和静息能量代谢、衰弱指标及透析患者症状负担调查表评分(DFSSBI)、汉密尔顿焦虑量表评分(HAMA)、汉密尔顿抑郁量表评分(HAMD)。结果 干预后观察组的代谢当量(METs)、峰值摄氧量(VO_(2)peak)、负荷运动时间(LOADEX)、正常步速、最大步速、静息代谢率(RMR)均高于对照组;衰弱评分降低、握力升高,DFSSBI量表各项评分均降低;HAMA和HAMD量表评分较对照组均降低,P均<0.05。结论 血液透析患者采用信息化多元运动干预能够提高心肺耐力,改善衰弱状态和症状负担,调整步速和静息能量代谢,改善负性情绪,具有一定的临床应用价值。Objective To analyze the effects of information-based multimodal exercise on exercise capacity and negative emotions in hemodialysis(HD)patients.Methods A total of 120 HD patients from Zhengzhou Central Hospital between June 2020 and June 2022 were recruited and randomly divided into a control group and an observation group,with 60 patients in each group.The control group received routine clinical interventions,while the observation group received information-based multimodal exercise interventions.The cardiorespiratory endurance,walking speed,resting energy metabolism,frailty indicators,Dialysis Fatigue Symptom Burden Inventory(DFSSBI)score,Hamilton Anxiety Scale(HAMA)score,and Hamilton Depression Scale(HAMD)score were compared between the two groups before and after the intervention.Results After the intervention,the metabolic equivalents(METs),peak oxygen consumption(VO 2peak),load exercise time(LOADEX),normal walking speed,maximum walking speed,and resting metabolic rate(RMR)in the observation group were higher than those in the control group.The frailty score decreased,and grip strength increased.All DFSSBI scores were reduced,and the HAMA and HAMD scores were lower than those in the control group,with all P values<0.05.Conclusion The application of information-based multimodal exercise interventions for HD patients can improve cardiorespiratory endurance,alleviate frailty and symptom burden,adjust walking speed and resting energy metabolism,and improve negative emotions,demonstrating clinical application value.
