后腹腔镜输尿管切开取石术治疗输尿管上段高位结石患者手术指标及一次性成功率并发症临床观察

摘　　要：目的探讨分析后腹腔镜输尿管切开取石术(RLU)治疗输尿管上段高位结石患者手术指标及一次性成功率、并发症临床观察。方法选取2020年5月至2023年5月在我院收治的80例输尿管上段高位结石患者,据手术方法不同将其分为对照组(实施输尿管镜碎石术)35例和RLU组(实施RLU)45例。分别比较2组患者的手术指标、一次性取石成功率、1个月结石清除率、炎症因子水平、术后并发症发生率。结果RLU组的手术时间、术中失血量、术后下床活动时间、住院时间、住院总费用较对照组缩短(P<0.05);RLU组的一次性取石成功率和1个月结石清除率为96%、98%较对照组的成功率和清除率80%、86%升高(P<0.05);手术后,RLU组炎症因子水平较对照组降低(P<0.05);对照组和RLU组并发症发生率分别为11%、4%,差异无统计学意义(P>0.05)。结论RLU治疗输尿管上段高位结石患者能缩短其手术相关时间,减少术中出血量,提高一次性成功率和结石清除率,降低炎症因子水平。Objective To investigate and analyze the clinical observation of operative indicators,one-time success rate,and complications of retroperitoneal ureterlithotomy(RLU)in the treatment of high calculi in the upper ureter.Methods A total of 80 patients with upper ureteral calculi admitted to our hospital from May 2020 to May 2023 were selected and divided into a control group(35 cases undergoing ureteroscopic lithotripsis)and an RLU group(45 cases undergoing retroperitoneal ureterolithotomy)according to different surgical methods.The surgical indexes,the success rate of one-time lithotomy,the stone clearance rate in one month,the level of inflammatory factors,and the incidence of postoperative complications were compared between the two groups.Results The operation time,intraoperative blood loss,postoperative activity time,hospital stay,and total hospitalization cost in the RLU group were shorter than those in the control group,and the difference was statistically significant(P<0.05).The success rate of one-time stone removal and one-month stone removal rate in the RLU group was 96%and 98%,which were higher than those in the control group(80%and 86%),and the difference was statistically significant(P<0.05).After the operation,the level of inflammatory factors in the RLU group was lower than that in the control group,and the difference was statistically significant(P<0.05).The incidence of complications in the control group and RLU group was 11%and 4%,respectively,and there was no significant difference(P<0.05).Conclusion RLU can shorten the operative time,reduce intraoperative blood loss,improve the one-time success rate and stone clearance rate,and reduce the level of inflammatory factors in patients with upper ureteral high calculi.
