秦艽丸联合刺络拔罐疗法治疗寻常型银屑病血热证的疗效评价及对血清炎症因子的影响

摘　　要：目的 探讨秦艽丸联合刺络拔罐疗法治疗寻常型银屑病血热证的临床疗效及对血清炎症因子的干预影响。方法 选取2021年10月至2023年10月该院皮肤科门诊收治的130例寻常型银屑病血热证患者随机分为治疗组和对照组各65例,治疗组给予中药汤剂秦艽丸加减联合中医特色外治刺络拔罐疗法治疗,对照组仅给予刺络拔罐疗法治疗,记录两组患者治疗前、第2周、第4周皮损PASI评分、生活质量DLQI指数评分、中医证候评分,并酶联免疫吸附实验(ELISA)法检测治疗前后患者血清中肿瘤坏死因子-α(TNF-α)、干扰素-γ(INF-γ)、白介素-6(IL-6)、白介素-17(IL-17)水平。结果 治疗组总有效率96.92%明显高于对照组83.08%(P<0.05),治疗后两组PASI评分、生活质量DLQI指数、中医证候评分明显降低,两组患者血清炎症因子TNF-α、INF-γ、IL-6、IL-17水平均较治疗前显著改善,且治疗组评分明显优于对照组,差异有统计学意义(P<0.05)。结论 秦艽丸联合刺络拔罐疗法可以有效改善寻常型银屑病血热证患者皮损状况及各项中医证候,降低炎症因子水平并提高患者生活质量,疗效确切,展现中医药特色治疗优势,值得临床推广使用。Objective To explore the clinical efficacy of Qinjiao Pill combined with pricking and cupping in the treatment of blood-heat syndrome of psoriasis and the interventional effects on serum inflammatory factors.Methods 130 patients with psoriasis with blood-heat syndrome who were admitted to the outpatient clinic of the dermatology department of the hospital from October 2021 to October 2023 were selected and randomly divided into 65 cases each in the treatment group and the control group.The treatment group was given Chinese herbal medicine soup Qinjiao Pill plus reduction combined with Chinese medicine characteristic external treatment pricking and cupping therapy.The control group was given only pricking and cupping therapy.PASI scores of skin lesions,DLQI index scores of quality of life,and scores of Chinese medicine evidence were recorded before treatment,at the 2nd week and at the 4th week in both groups,and the levels of tumor necrosis factor-α(TNF-α),interferon-gamma(INF-),interleukin-6(IL-6),and interleukin-17(IL-17)were detected by ELISA before and after the treatment in patientsblood serum.Results The total effective rate of 96.92%in the treatment group was significantly higher than 83.08%in the con-trol group(P<0.05).The PASI score,quality of life DLQI index,and Chinese medicine evidence score of the two groups were significantly reduced after treatment.The levels of serum inflammatory factors TNF-α,INF-,IL-6 and IL-17 in both groups were significantly improved compared with those before treatment,and the scores of the treatment group were significantly better than those of the control group,with statistically significant differences(P<0.05).Conclusion Qinjiao Pill combined with pricking and cupping can effectively improve the skin lesions and Chinese medicine symptoms of patients with psoriasis with blood-heat syndrome,reduce the level of inflammatory factors and improve the quality of life of patients.The therapeutic effect is accurate,showing the characteristic therapeutic advantages of traditional
