“降落伞”式固定肱骨大小结节在人工肱骨头置换术中的疗效评价

摘　　要：目的:探讨“降落伞”式固定大小结节在人工肱骨头置换术中的近期临床疗效。方法:回顾性分析2018年1月至2021年1月行人工肱骨头置换术治疗的老年肱骨近端骨折患者25例,收集术后并发症、Constant-Murley肩关节功能量表(CMS)评分、疼痛视觉模拟量表(VAS)评分、末次随访肩关节活动度等数据并进行统计分析。结果:25例患者均获随访,随访时间为18~36个月,平均随访时间为24.12个月。末次随访未出现假体松动、假体脱位、大小结节移位、异位骨化等并发症。CMS评分:优5例,良18例,可1例,差1例,优良率为92%。有1例患者出现活动时肩关节轻微疼痛,可耐受。肩关节活动度:外展95°~130°,平均110.52°;前屈100°~135°,平均127.20°;外旋30°~45°,平均37.56°;内旋40°~65°,平均55.16°。结论:“降落伞”式固定大小结节在老年肱骨近端骨折肱骨头置换术中取得较为满意的临床疗效,肩关节功能恢复满意,大小结节未出现继发性移位。Objective:To explore the short-term clinical efficacy of“falling parachute”type fixed-size humeral nodules in artificial humeral head replacement.Methods:Retrospective analysis of 25 cases of proximal humeral fractures in elderly patients treated with artificial head replacement between January 2018 and January 2021.Postoperative complications,Constant-Murley shoulder scale(CMS)score,visual analogue scale(VAS)score,and last follow-up shoulder motion data were collected and analyzed for statistical significance.Results:All 25 patients were followed up for 1836 months,with an average follow-up time of 24.12 months.At the last follow-up,there were no complications such as prosthesis loosening,dislocation,displacement of nodules and heterotopic ossification.CMS score:excellent 5 cases,good 18 cases,fair 1 case,poor 1 case,excellent and good rate 92%.There was 1 patient with mild shoulder joint pain during movement,which was tolerable.Shoulder range of motion:abduction 95°130°,average 110.52°;forward bend 100°135°,average 127.20°;external spin 30°45°,average 37.56°;internal spin 40°65°,average 55.16°.Conclusion:In humeral head replacement for fractures of the proximal humerus in the elderly,the“falling parachute”type fixed-size humeral nodules achieved satisfactory clinical results,satisfactory return of shoulder function,and no secondary displacement of the nodules.
