摘　　要：持续“非可控性炎症”成为结直肠炎-癌转化的关键因素,可表现为持续或过度的变态反应导致免疫细胞亚群及炎症因子的改变,即所谓的免疫不平衡状态。结直肠炎-癌转化过程中,脾虚为本,湿热瘀毒为标,湿、热、瘀、痰等邪日久蕴结化毒,故脾虚毒蕴是结直肠炎-癌转化的关键。故在治疗上当以脾虚及毒邪的强弱变化为主线,根据各阶段的表现辨证论治,把阶段辨证与整体论治相结合。健脾益气中药可通过改善结直肠肿瘤细胞的慢性炎症和能量代谢障碍,调控肿瘤微环境,延缓肿瘤细胞上皮间质转化过程,抑制结直肠肿瘤细胞的生长和转移。解毒类中药亦可通过调控免疫功能、缓解炎症反应、抑制血管生成等多途径,控制肿瘤的进展及转移。Persistent"uncontrolled inflammation"has become a key factor in the inflammation-to-cancer transition,which can be manifested as persistent or excessive allergic reactions leading to changes in immune cell subsets and inflammatory factors,the so-called immune imbalance.In the process of inflammation-to-cancer transition,the Spleen deficiency is the root while the Dampness,Heat,stasis and Toxin are being the treetop.The Toxin is induced by accumulation of Evil factors such as Dampness,Heat,stasis and Phlegm for a long time.From this prospect,Spleen deficiency with Toxin accumulation is the key to transition from colitis to colorectal cancer.Therefore,the treatment of the disease should follow the guideline of strength contrast between Spleen deficiency and Toxin factors,also be based on syndrome differentiation according to the manifestations of each stage,combining the phased syndrome differentiation with the overall treatment.Traditional Chinese medicine for invigorating Spleen and benefiting Qi can regulate the tumor microenvironment by improving the chronic inflammation and energy metabolism disorders of colorectal tumor cells,inhibit the epithelial-mesenchymal transition process of tumor cells,as well as improving the growth and metastasis of colorectal tumor cells.Traditional Chinese medicine for resolving Toxin can also participate in the development and metastasis of intestinal tumors by regulating immune function,relieving inflammatory reaction,as well as inhibiting angiogenesis and other ways.
