基于MYMOP半结构化量表探讨以症状为主体的糖尿病足中医药疗效评价方法的构建与改进

摘　　要：糖尿病足是一种病机复杂且病情严重的糖尿病并发症。在中医干预糖尿病足的临床研究中,如何在科学评价症状改善的同时反映中医药的疗效特色是构建糖尿病足中医疗效评价方法的关键问题。结合糖尿病足的疾病特征,将基于患者报告的症状量表(measure yourself medical outcome profile,MYMOP)应用于中医药治疗糖尿病足的症状疗效评价,可一定程度改善现有的评价方法对糖尿病足患者全身症状、生活质量、身心状态等评价的不足。在此基础上,针对MYMOP量表应用于糖尿病足中医疗效评价存在的不足进行相应改进,如采用将糖尿病足局部与全身症状分开进行MYMOP评价、在MYMOP量表的半结构化基础上增加聚焦糖尿病足症状改善的题项以及将MYMOP与中医证候积分相结合等方法,将使其更适用于中医药治疗糖尿病足的疗效评价。Diabetic foot is a serious complication of diabetes with complex pathogenesis.In the clinical research of TCM intervention on diabetic foot,how to scientifically evaluate the improvement of symptoms and reflect the efficacy characteristics of TCM is the key issue to construct the evaluation method of TCM efficacy on diabetic foot.Combined with the disease characteristics of diabetic foot,the measure yourself medical outcome profile(MYMOP)was applied to evaluate the symptom efficacy of TCM in the treatment of diabetic foot.To some extent,it can improve the shortcomings of the existing evaluation methods in the evaluation of systemic symptoms,quality of life,as well as physical and mental state of patients with diabetic foot.On that basis,corresponding improvements were made to the shortcomings of MYMOP scale in the evaluation of TCM efficacy on diabetic foot,such as methods of using MYMOP evaluation separately for local and systemic symptoms of diabetic foot,adding items focused on the improvement of diabetic foot symptoms on the semi-structured basis of MYMOP scale,and combining MYMOP with TCM syndrome score so as to make it more suitable for the evaluation of the efficacy of traditional Chinese medicine in the treatment of diabetic foot.
