摘　　要：恶性腹腔积液主要涉及肝、脾、肾等脏腑。肝失疏泄,气血瘀滞,脾失健运,水湿内停,肾失温煦,寒水内结是关键因素,致气滞、血瘀、水停于腹,久则正气衰败。治疗当以温阳健脾补肾为本,调气行血为先,行气理血肃清水道,温补阳气以助气化,助脾健运以行水湿,温肾复阳以利水行,辅以疏肝,固护胃气,分消水液。用药以甘温味辛为主,取甘温助阳、辛可散行之义,常用方药如五苓散、真武汤等,亦可酌加延胡索、乌药、川花椒等理气活血之品。Malignant peritoneal effusion mainly involves Liver,Spleen,Kidney and other Zang and Fu.Liver Qi discharge dysfunction,Qi and blood stagnation,Spleen dysfunction,internal stagnation of Water and Dampness,Kidney Yang dificiency,and internal knots of cold Water are the key factors of the disease,causing Qi stagnation,blood stasis,Water stagnation in the abdomen,and the decay of Zheng Qi for a long time.Therefore,in clinical treatment,warming Yang,tonifying the Spleen and strengthening the Kidney should be the basic policy,regulating Qi and promoting blood circulation should be the first step.By regulating Qi and blood and clearing the waterways as well as warming and replenishing Yang Qi to help Qihua,helping Spleen to relieve Water and Dampness,warming the Kidney and strengthening Yang to facilitate Water flow.It is supplemented by soothing the Liver,consolidating Stomach Qi and dissipating Water.The medicine is mainly sweet and warm and pungent,because sweet and warm drugs can help Yang while pungent drugs can promote Qi movement.The commonly used prescriptions are Wuling Powder and Zhenwu Decoction,etc.,and Yanhusuo(corydalis tuber),Wuyao(lindera aggregata)and Chuanhuajiao(fructus zanthoxyli)and other products,which function in regulating Qi and invigorating blood circulation,can be added accordingly..
