王祖龙从“藏泄平衡观”探讨早泄辨治思路

摘　　要：临床辨治早泄应立足于中医整体观,重视脏腑之间的动态平衡,不应拘泥于某一脏腑。王祖龙教授从平衡论出发,对脏腑中蕴含着的藏泄关系进行概括,认为早泄的发生与藏泄平衡关系密切,将其概括为藏泄平衡观。基于该观点将早泄分为疏泄过度和封藏不足,辨证为肝阳上亢证、肝经郁热证、肾气不足证、阴虚火旺证、湿热内蕴证、瘀血阻滞六证,并以“平肝固精”“清肝解郁”“补肾抑肝”“滋阴清火”“清热燥湿”“化瘀固精”六法治疗。王祖龙通过调理藏泄平衡论治早泄,立意独到,组方善用药对增强方剂疗效,丰富了中医对早泄的认识,为临床治疗男性疾病提供了新的思路。The clinical treatment of premature ejaculation should be based on the holistic view of traditional Chinese medicine,paying attention to the dynamic balance between the Zang and Fu,and should not be confined to a certain one.Starting from the theory of balance,Professor Wang Zulong summarized the relationship between storage and discharge contained in the Zang and Fu and believed that the occurrence of premature ejaculation is closely related to the balance of storage and discharge,which is summarized as the concept of balance between storage and discharge.Based on this viewpoint,premature ejaculation is divided into excessive excretion and insufficient sealing,and the syndrome differentiation can be hyperactive Liver Yang,depressed Heat in Liver meridian,Kidney Qi deficiency,Yin deficiency induced Fire,Dampness-Heat internal accumulation,and blood stasis blockage syndromes,which can be treated with the following six methods respectively:"Harmonizing the Yin and Yang of Liver to consolidate Essence","Clearing Liver Fire and relieving depression","Tonifying the Kidney and inhibiting the Liver","nourishing Yin and clearing Fire","Clearing Heat and relieving Dampness",and"dissolving stasis and consolidating Essence".It′s a unique idea that regulating the balance of storage and discharge to treat premature ejaculation.Besides,couplet drugs are skillfully used in the prescription to enhance the efficacy of the formula.Both enriches the understanding of traditional Chinese medicine on premature ejaculation and provides a new idea for the clinical treatment of male diseases.
