龙胆泻肝汤对原发性肝癌肝动脉栓塞化疗术后患者肝功能及肿瘤标志物的影响研究

摘　　要：目的:探讨龙胆泻肝汤加减对原发性肝癌(PHC)肝动脉栓塞化疗术(TACE)后患者肝功能及肿瘤标志物的影响效果。方法:选取100例PHC TACE术后患者,按随机抽签法分为常规西医对症治疗的对照组和联合龙胆泻肝汤加减的治疗组,每组各50例,两组均治疗8周。比较两组临床疗效、中医症候积分、肝功能、肿瘤标志物水平、不良反应发生情况。结果:治疗8周后,治疗组总有效率高于对照组(P<0.05)。治疗8周后,两组恶心呕吐、肝脏肿块、胁肋疼痛、大便溏泄评分,血清谷丙转氨酶(ALT)、谷草转氨酶(AST)、总胆红素(TB)、谷氨酰转肽酶(GTT)水平及血清癌胚抗原(CEA)、胸苷激酶1(TK1)、甲胎蛋白(AFP)、糖类抗原19-9(CA19-9)水平均低于治疗前,治疗组低于对照组(P<0.05)。治疗8周后,两组血清白蛋白(ALB)水平均高于治疗前,治疗组高于对照组(P<0.05)。治疗期间,两组不良反应比较差异无统计学意义(P>0.05)。结论:龙胆泻肝汤加减治疗可有效改善PHC TACE术后患者临床疗效、肝功能,降低肿瘤标志物水平,具有较好的治疗效果。Objective:To investigate the effect of Longdan Xiegan decoction on liver function and tumor markers in patients with primary liver cancer(PHC)after transcatheter arterial chemoembolization(TACE).Methods:A total of 100 patients with PHC after TACE were randomly divided into the control group treated with conventional western medicine and the treatment group treated with Longdan Xiegan decoction,50 cases in each group.Both groups were treated for 8 weeks.The clinical efficacy,traditional Chinese medicine syndrome score,liver function,tumor marker level and adverse reactions were compared between the two groups.Results:After 8 weeks of treatment,the total effective rate of the treatment group was higher than that of the control group(P<0.05).After 8 weeks of treatment,the scores of nausea and vomiting,liver mass,hypochondriac pain,loose stool,levels of serum alanine aminotransferase(ALT),aspartate aminotransferase(AST),total bilirubin(TB),glutamyl transpeptidase(GTT)and serum carcinoembryonic antigen(CEA),thymidine kinase 1(TK1),alpha fetoprotein(AFP),carbohydrate antigen 19-9(CA19-9)levels in the two groups were lower than those before treatment,and those in the treatment group were lower than the control group(P<0.05).After 8 weeks of treatment,the level of serum albumin(ALB)in the two groups were higher than those before treatment,and the treatment group was higher than the control group(P<0.05).During the treatment,there was no significant difference in adverse reactions between the two groups(P>0.05).Conclusion:Longdan Xiegan decoction can effectively improve the clinical efficacy and liver function of patients with PHC after TACE,and reduce the level of tumor markers.It had good therapeutic effect and good safety.
