温肺化饮汤治疗慢性阻塞性肺疾病急性加重期疗效及对患者气道重塑和CTRP-4、CTRP-5水平的影响

摘　　要：目的:探究温肺化饮汤治疗寒饮停肺型慢性阻塞性肺疾病急性加重期(AECOPD)的疗效,并分析其对患者气道重塑指标及血清补体C1q肿瘤坏死因子相关蛋白-4(CTRP-4)、CTRP-5水平的影响。方法:将102例AECOPD患者随机分为两组。对照组51例予以常规西药治疗,观察组51例在此基础上加服温肺化饮汤。评价两组患者临床疗效及中医症候积分,比较两组治疗前后的气道重塑指标[基质金属蛋白酶-2(MMP-2)、基质金属蛋白酶-9(MMP-9)、神经生长因子(NGF)]及CTRP-4、CTRP-5水平,观察两组的不良反应情况。结果:观察组的临床疗效高于对照组(P<0.05);相较于对照组,观察组治疗后的各中医症候积分降低(P<0.05);与对照组比较,观察组治疗后的血清MMP-2、MMP-9、NGF、CTRP-4、CTRP-5水平均降低(P<0.05);两组不良反应情况组间比较差异无统计学意义(P>0.05)。结论:温肺化饮汤治疗寒饮停肺型AECOPD患者的疗效显著,可有效缓解临床症状,并可通过调节气道重塑指标及CTRP-4、CTRP-5水平发挥作用。Objective:To explore the efficacy of Wenfei Huayin decoction in the treatment of cold fluid retention in the lung type of acute exacerbation of chronic obstructive pulmonary disease(AECOPD),and to analyze its influence on airway remodeling indexes,serum complement C1q tumor necrosis factor-related protein-4(CTRP-4)and CTRP-5 levels.Methods:Totally 102 patients with AECOPD were randomized into two groups.51 cases in the control group were treated with conventional western medicine,while 51 cases in the observation group were added with Wenfei Huayin decoction on this basis.The clinical efficacy and TCM symptoms scores were evaluated in the two groups.The airway remodeling indexes[matrix metalloproteinase-2(MMP-2),matrix metalloproteinase-9(MMP-9),nerve growth factor(NGF)],CTRP-4 and CTRP-5 were compared before and after treatment,and the adverse reactions were observed.Results:The observation group had higher clinical efficacy(P<0.05).The scores of TCM symptoms were decreased in the observation group after treatment(P<0.05).In the observation group,serum levels of MMP-2,MMP-9,NGF,CTRP-4 and CTRP-5 after treatment were declined compared to the control group(P<0.05).There were no obvious differences in adverse reactions between groups(P>0.05).Conclusion:Wenfei Huayin decoction is effective in the treatment of AECOPD of cold fluid retention in the lung.It can effectively relieve clinical symptoms and play a role by regulating airway remodeling indexes and CTRP-4 and CTRP-5 levels.
