基于路径化管理的慢病患者围手术期抗血栓药物管理效果评价

摘　　要：目的评价药师基于路径化管理对慢病患者围手术期抗血栓药物管理效果。方法选取2022年3月~2024年3月云浮市人民医院收治的160例慢病患者作为研究对象,按照随机数字表法将患者分为对照组和观察组两组,每组各80例。对照组实施围手术期抗血栓药物常规管理,观察组在对照组的基础上增加药师基于路径化管理。对比两组患者下肢深静脉血栓(DVT)发生率、心脑血管病发生率、住院时间、住院费用和用药依从性。结果观察组患者的DVT发生率和心脑血管疾病发生率低于对照组,差异有统计学意义(P<0.05);观察组患者的住院时间和住院费用比对照组更低,差异有统计学意义(P<0.05);观察组患者的用药依从性比对照组更高,差异有统计学意义(P<0.05)。结论慢病患者围手术期的抗血栓药物管理时,药师基于路径化管控的实施能减少患者DVT的发生,缩短住院时间,减少住院费用,提高患者的用药依从性。Objective To evaluate the effect of pharmacists based on routing management on perioperative antithrombotic drug management in chronic patients.Methods A total of 160 patients with chronic diseases admitted to Yunfu People's Hospital from March 2022 to March 2024 were selected as the study objects.According to the random number table method,the patients were divided into the control group and the observation group with 80 cases in each group.Routine management of perioperative antithrombotic drugs was implemented in the control group,and the observation group was given routing management by pharmacists on the basis of the control group.The incidence of deep vein thrombosis(DVT),incidence of cardiovascular and cerebrovascular disease,hospitalization time,hospitalization cost and medication compliance were compared between the two groups.Results The incidence of DVT and cardiovascular and cerebrovascular disease in the observation group was lower than that in the control group,the difference was statistically significant(P<0.05).The hospitalization time and hospitalization cost in the observation group were lower than those in the control group,the difference was statistically significant(P<0.05).The medication compliance of the observation group was higher than that of the control group,and the difference was statistically significant(P<0.05).Conclusion In perioperative management of antithrombotic drugs in patients with chronic diseases,the implementation of pharmacists'routing management can reduce the incidence of DVT,shortening the length of stay and reducing hospitalization costs,and improve the medication compliance of patients.
