摘  要:目的定量分析超声射频(RF)信号特征参数与甲状腺结节良恶性之间的关系,探讨多种RF信号参数在甲状腺肿瘤良恶性之间的差异及其鉴别中国甲状腺影像报告和数据系统(C-TIRADS)评分4类甲状腺结节良恶性的价值。方法收集行甲状腺结节细针穿刺的患者,以细针穿刺病理结果作为甲状腺结节良恶性诊断依据,提取RF信号,重构二维图像后获取结节感兴趣区(ROI)及其特征参数,并分析特征参数与结节良恶性之间的相关性。结果共161例患者行超声引导下细针穿刺,187个穿刺结节纳入此次研究,单因素分析显示RF信号参数中斜率(Slope)、截距(Intercept)、中频带(Mid-band)、S1、S2与甲状腺结节的良恶性独立相关,Slope、Intercept、Mid-band、S1、S2的受试者工作特征曲线下面积(AUC)分别为0.59、0.63、0.63、0.66、0.60,对甲状腺结节良恶性的诊断有重要的参考意义。结论RF信号参数在甲状腺肿瘤良恶性之间具有差异性,可以提供更丰富的组织信息,在甲状腺结节良恶性的诊断中具有重要的应用价值,可以有效减少不必要的甲状腺结节细针穿刺。Objective To quantitatively analyze the relationship between ultrasound radio frequency(RF)signal parameters and the benign and malignant nature of thyroid nodules,and to investigate the differences in multiple ultra-sound RF signal parameters between benign and malignant thyroid tumors and their value in differentiating the benign and malignant nature of Chinese thyroid imaging reporting and data system(C-TIRADS)score 4 category thyroid nod-ules.Methods Patients who underwent fine needle aspiration of thyroid nodules were included.Pathological results from fine needle aspiration were used as the diagnostic basis for determining the benign and malignant nature of the thyroid nodules.Ultrasound RF signals were collected,and after reconstructing two-dimensional images,the region of interest(ROI)of the nodules and their feature parameters were extracted for analysis of their correlation with the be-nign and malignant nature of the nodules.Results A total of 161 patients underwent ultrasound-guided fine needle as-piration,with 187 punctured nodules included in this study.Univariate analysis showed that ultrasound RF signal pa-rameters including Slope,Intercept,Mid-band,S1,and S2 were independently associated with the malignancy of thy-roid nodules.The area under the curve(AUC)values for Slope,Intercept,Mid-band,S1,and S2 were 0.59,0.63,0.63,0.66,and 0.60,respectively,indicating their significant diagnostic value for determining the benign and malig-nant nature of thyroid nodules.Conclusions Ultrasound RF signal parameters demonstrate differences in the malig-nancy of thyroid tumors,offering richer tissue information and holding important application value in the diagnosis of benign and malignant thyroid nodules,potentially reducing unnecessary fine needle aspiration of thyroid nodules.
