摘 要:目的评估中、重度肺动脉高压(PH)患者肺动脉硬化度及其与右心导管测值的相关性。方法前瞻性选取经过右心导管检查确诊的29例中、重度PH患者,同期来宁夏医科大学总医院就诊的健康者为对照组,利用超声心动图评估肺动脉硬化度(PAS);右心导管测量肺动脉收缩压(PASP)、肺动脉舒张压(PADP)、肺动脉平均压(mPAP)、肺毛细血管楔压(PCWP)、右心室收缩末压(RVSP)、右心室舒张末压(RVDP)、右心室平均压(mRVP);并分析PAS与右心导管测值相关性。结果与对照组相比,中度PH组和重度PH组的PAT减低,PAS增高,差异具有统计学意义,与中度PH组相比,重度PH组PAT减低,PAS增高(P<0.05);与中度PH组相比,重度PH组PASP、PADP、mPAP、RVDP、RVSP、mRVP等右心导管测值增高(P<0.05);中、重度PH组的PAS与右心导管测值的相关性分析显示:PAS与PASP、PADP、mPAP、RVSP、RVDP、mRVP呈正相关关系。结论通过超声心动图获得的PAS指标评估肺动脉压力具有一定的可行性,为评估肺动脉压力提供一个新的思路。Objective To assess the degree of pulmonary artery stiffness and its correlation with right heart cath-eterization measurements in patients with moderate and severe pulmonary hypertension(PH).Methods Twenty-nine patients with moderate and severe PH diagnosed after right heart catheterization were prospectively selected as the study group,and healthy subjects visiting General Hospital of Ningxia Medical University during the same period were enrolled as the control group.The degree of pulmonary artery stiffness(PAS)was assessed using echocardiography;pulmonary artery systolic pressure(PASP),pulmonary artery diastolic pressure(PADP),mean pulmonary artery pressure(mPAP),pulmonary capillary wedge pressure(PCWP),right ventricular end-systolic pressure(RVSP),right ventricular end-diastolic pressure(RVDP),and mean right ventricular pressure(mRVP)were measured with right heart catheterization;and the correlation between the PAS and the measured values of right heart catheterization was analyzed with right heart catheterization measurements correlation.Results Compared with the control group,PAT was reduced and PAS was increased in the moderate PH and severe PH groups,and the difference was statisti-cally significant.Compared with the moderate PH group,PAT was reduced and PAS was increased in the severe PH group(P<0.05);compared with the moderate PH group,right heart catheterization measurements of PASP,PADP,mPAP,RVDP,RVSP,and mRVP were increased in the severe PH group(P<0.05);correlation analysis between PAS and right heart catheterization values in the moderate and severe PH groups showed a positive correlation between PAS and PASP,PADP,mPAP,RVSP,RVDP,and mRVP.Conclusions The PAS index obtained by echo-cardiography is feasible to assess pulmonary artery pressure and provides a new idea to assess pulmonary artery pres-sure.
