常规超声及超声造影在胰腺囊肿和囊性肿瘤鉴别诊断中的研究

摘　　要：目的探讨常规超声及超声造影(CEUS)在胰腺囊肿和胰腺囊性肿瘤(PCNs)鉴别诊断中的价值。方法回顾性分析32例胰腺囊肿和96例PCNs的常规超声及CEUS声像图特征,发现具有鉴别诊断价值的声像图特征。结果常规超声声像图特征比较中,胰腺囊肿和PCNs在位置、大小、内部成分、有无分隔之间的差异具有统计学意义(P<0.05),二者在边界、形态、主胰管有无扩张的差异无统计学意义(P>0.05)。CEUS声像图特征比较中,二者在有无分隔、壁结节样强化、病变是否与主胰管相通之间的差异具有统计学意义(P<0.05),二者在内部成分、动脉期及静脉期强化模式之间的差异无统计学意义(P>0.05)。结论胰腺囊肿和PCNs的常规超声及CEUS声像图之间存在差异,常规超声及CEUS在二者的鉴别诊断中具有一定价值。Objective To investigate the value of conventional ultrasound and contrast-enhanced ultrasound(CEUS)in the differential diagnosis of pancreatic cysts and pancreatic cystic neoplasms(PCNs).Methods Thirty-two cases of pancreatic cysts and 96 cases of PCNs were retrospectively analyzed by conventional ultrasound and CEUS,and the ultrasonographic features with differential diagnosis value were explored.Results There were signifi-cant differences in the location,size,internal composition and septa between the pancreatic cysts and PCNs(P<0.05).There were no significant differences in the boundary,shape and main pancreatic duct dilatation between the two groups(P>0.05).In the comparison of CEUS sonographic features,there were statistically significant differ-ences between the two groups in the presence or absence of septa,nodular enhancement of the cyst wall,and whether the lesion was communicated with the main pancreatic duct(P<0.05).There was no significant difference between the two groups in the internal components,arterial and venous phase enhancement(P>0.05).Conclusions There are differences between conventional ultrasound and CEUS images of pancreatic cysts and PCNs.Conventional ultra-sound and CEUS have certain value in the differential diagnosis of the two diseases.
