超声造影诊断卵巢囊肿蒂扭转的应用研究

摘　　要：目的探讨超声造影诊断卵巢囊肿蒂扭转的价值。方法选取临床怀疑卵巢囊肿蒂扭转的患者65例,均行超声造影检查。将患者分为扭转组和非扭转组,比较两组造影增强模式的差异,观察指标为开始增强时间、达峰时间、峰值强度。以晚增强、晚达峰、低增强(包括无增强)作为诊断卵巢囊肿蒂扭转的诊断指标,采用受试者工作特征(ROC)曲线评估超声造影对卵巢囊肿蒂扭转的诊断价值。结果65例卵巢囊肿中扭转24例,未扭转41例。ROC曲线显示晚增强、晚达峰、低增强的曲线下面积(AUC)分别为0.951、0.997、0.950(P<0.001),诊断卵巢囊肿蒂扭转的灵敏度分别为87.50%、91.67%、91.67%,特异度分别为97.56%、97.56%、87.80%。结论超声造影在卵巢囊肿蒂扭转中有很高的诊断价值,可以提高术前诊断的准确度。Objective To investigate the value of contrast-enhanced ultrasound(CEUS)in the diagnosis of ovari-an cysts torsion.Methods A total of 65 patients who suspected ovarian cysts torsion underwent contrast-enhanced ul-trasound examination were selected.The patients were divided into the torsion group and the non-torsion group,and the differences of contrast enhancement patterns were compared between torsion group and non-torsion group with the initial enhancement time,time to peak,and peak intensity as indications.The late enhancement,late time to peak,and low enhancement were served as the diagnostic criterion of ovarian cysts torsion.The receiver operating character-istic(ROC)curve was used to evaluate the diagnostic efficacy of CEUS for ovarian cyst torsion.Results There were 24 patients in torsion group and 41 patients in non-torsion group.The ROC curve showed that the area under curve(AUC)of late enhancement,late time to peak and low enhancement were 0.951,0.997 and 0.950 respectively(P<0.001).The sensitivities of ovarian cyst torsion diagnosis were 87.50%,91.67%,and 91.67%respectively,and the specificities were 97.56%,97.56%,and 87.80%respectively.Conclusions CEUS appears high diagnostic value and can improve the accuracy of preoperative diagnosis in ovarian cysts torsion.
