腮腺沃辛瘤样黏液表皮样癌的超声表现和临床病理特点

摘　　要：目的探讨腮腺沃辛瘤样黏液表皮样癌(WT-MEC)的临床病理特点、超声表现及其诊断价值。方法回顾性分析4例病理证实为WT-MEC患者的临床、超声及病理资料。结果4例WT-MEC均为单发病灶,声像图均表现为边界清晰、形态规则、后方回声增强的囊实性结节;囊性为主3例,实性为主1例;4例结节均于内部或周边探及Ⅰ级血流信号。4例均未见转移性淋巴结,术后随访均无复发或转移。免疫组化示Ki-67平均阳性率为5.3%。结论WT-MEC为低度恶性涎腺肿瘤,缺乏典型恶性肿瘤的声像图特征,结合临床特点可能有助于鉴别诊断,其确诊仍需依赖病理检查。Objective To investigate the clinicopathological features,ultrasound manifestations and diagnostic value of ultrasonography in Warthin-like mucoepidermoid carcinoma(WT-MEC)of the parotid gland.Methods The clinical,ultrasonographic and pathological data of four patients with WT-MEC which confirmed by pathology were ret-rospectively analyzed.Results All four cases of WT-MEC were solitary lesions,and the sonographic images all showed cystic-solid nodules with clear boundaries,regular shape,and posterior acoustic enhancement.Three cases were predominantly cystic,and one case was predominantly solid.All four nodules exhibited gradeⅠblood flow signal internally or peripherally.No metastatic lymph nodes were observed in any of the cases,and there were no recurrences or metastases during the postoperative follow-up.Immunohistochemistry showed an average Ki-67 positivity rate of 5.3%.Conclusions WT-MEC is a low-grade malignant salivary gland tumor lacking the typical sonographic features of malignant tumors.Combining clinical characteristics may aid in the differential diagnosis,but definitive diagnosis still relies on pathological examination.
