固尔苏和珂立苏治疗NRDS的疗效及对患儿血清TGF-β1、BMP-7水平的影响

摘  要:目的探讨固尔苏和珂立苏治疗新生儿呼吸窘迫综合征(NRDS)的疗效及对患儿血清转化生长因子β1(TGF-β1)、骨形态发生蛋白-7(BMP-7)水平的影响,以期为NRDS患儿的治疗提供参考。方法选取2019年10月-2023年11月马鞍山市妇幼保健院新生儿科收治的120例NRDS患儿,采用区组随机化法分为固尔苏组和珂立苏组,每组60例。2组患儿均经微管气管内注入肺表面活性物质(LISA)治疗,其中固尔苏组的肺表面活性物质为固尔苏,珂立苏组则注入珂立苏治疗。比较2组患儿的治疗效果和治疗期间的不良反应及并发症发生情况。同时,比较2组患儿治疗前和治疗24 h后的血清细胞因子(TGF-β1和BMP-7)和炎症因子[降钙素原(PCT)和C-反应蛋白(CRP)]水平,并比较2组患儿治疗后的新生儿神经行为评分(NBNA)。结果2组患儿的给药时间、氧疗天数和住院时间比较,差异均无统计学意义(P>0.05);珂立苏组患儿的住院费用低于固尔苏组,差异有统计学意义(P<0.05)。2组患儿治疗期间的不良反应及并发症发生率比较,差异无统计学意义(P>0.05)。治疗前,2组患儿血清TGF-β1和BMP-7水平比较,差异均无统计学意义(P>0.05);治疗后,2组患儿血清TGF-β1和BMP-7水平均低于治疗前,差异均有统计学意义(P<0.05),但组间比较,差异均无统计学意义(P>0.05)。治疗前,2组患儿血清PCT和CRP水平比较,差异均无统计学意义(P>0.05);治疗后,2组患儿血清PCT和CRP水平均低于治疗前,差异均有统计学意义(P<0.05),但组间比较,差异均无统计学意义(P>0.05)。2组患儿治疗1周后NBNA量表的新生儿行为能力、被动肌张力、主动肌张力、原始反射及一般状况5个维度评分比较,差异均无统计学意义(P>0.05)。结论固尔苏和珂立苏治疗NRDS患儿的疗效和安全性相当,均能有效改善患儿血清TGF-β1、BMP-7水平、炎症因子水平和神经行为发育状况。临床可根据患儿的具体情况和需要采用�Objective To explore the efficacy of Curosurfact and Colistin in the treatment of neonatal respiratory distress syndrome(NRDS)and their impact on the levels of transforming growth factor-beta 1(TGF-β1)and bone morphogenetic protein-7(BMP-7)in infants,to provide evidence for the treatment of NRDS in infants.Methods A total of 120 infants with NRDS admitted to the Department of Neonatology of our hospital,from October 2019 to November 2023,were selected as the participants for the study.They were assigned to the Curosurfact group and the Colistin group by block randomization,with 60 cases in each group.Both groups of infants were treated with minimally invasive percutaneous intratracheal instillation of lung surfactant(LISA),with the Curosurfact group receiving Curosurfact and the Colistin group receiving Colistin.The therapeutic efficacy and adverse reactions and complications during the treatment of the two groups were compared.Meanwhile,the levels of serum cytokines(TGF-β1 and BMP-7)and inflammatory factors[procalcitonin(PCT)and C-reactive protein(CRP)]before and 24 hours after treatment in both groups were compared,as well as the neonatal behavioral neuro-assessment(NBNA)scores after treatment in both groups.Results No statistically significant differences were observed in the medication time,oxygen therapy days,and length of hospital stay between the two groups of infants(P>0.05).The hospitalization costs of the Colistin group were significantly lower than those of the Curosurfact group(P<0.05).No statistically significant difference was found in the incidence of adverse reactions and complications during treatment between the two groups(P>0.05).Before treatment,no statistically significant difference was noted in the levels of serum TGF-β1 and BMP-7 between the two groups(P>0.05).After treatment,the levels of serum TGF-β1 and BMP-7 in both groups were significantly lower than before treatment(P<0.05),but there were no statistically significant differences between the groups(P>0.05).Before treatment,there
