四步高位灌肠法对早产儿胎粪排出延迟的效果观察

摘　　要：目的探讨四步高位灌肠法对早产儿胎粪排出延迟的应用效果。方法便利选取2020年11月—2023年4月在陕西省某三级甲等综合医院新生儿病区住院治疗的63例胎粪排出延迟的早产儿为研究对象,采用随机数字表法将其分为对照组31例和观察组32例。对照组早产儿采用常规灌肠方法,观察组早产儿采用四步高位灌肠法。观察比较2组早产儿灌肠有效率、首次排粪质量、胎粪排尽时间和灌肠总次数。结果观察组灌肠有效率较对照组高(P<0.05),且首次排粪质量多于对照组(P<0.001),观察组早产儿胎粪排尽时间短于对照组(P<0.05),灌肠总次数少于对照组(P<0.001)。结论应用四步高位灌肠法可以提高胎粪排出延迟早产儿的灌肠有效率,增加首次排粪质量,缩短胎粪排尽时间,减少灌肠总次数。Objective To investigate the effect of four-step high enema on delayed meconium discharge in premature infants.Methods A total of 63 premature infants with delayed meconium discharge who were hospitalized in the neonatal ward of a tertiary grade-A general hospital in Shanxi Province from November 2020 to April 2023 were selected as the research objects.They were divided into control group(n=31)and observation group(n=32)by random number table method.Routine enema was performed in the control group,and four-step high enema in the observation group.The effective rate of enema,the quality of first defecation,the time of exhausting meconium and the total number of enema were observed and compared between the two groups.Results The effective rate of enema in the observation group was higher than that in the control group(P<0.05),and the quality of first defecation in the observation group was significantly higher than that in the control group(P<0.001).The time of meconium evacuation in the observation group was shorter than that in the control group(P<0.05),and the total number of enema in the observation group was significantly less than that in the control group(P<0.001).Conclusion The application of four-step high enema can improve the effective rate of enema in premature infants with delayed meconium discharge,increase the quality of first defecation,shorten the time of complete meconium discharge,and reduce the total number of enema.
