绿激光汽化联合围手术期提肛肌训练治疗良性前列腺增生的效果及对性功能的影响

摘　　要：目的探讨绿激光汽化联合围手术期提肛肌训练治疗大体积良性前列腺增生(benign prostatic hyperplasia,BPH)的效果。方法选择2022年1月—2023年6月东莞市中西医结合医院泌尿外科收治的60例大体积BPH患者作为观察对象。采取随机数字表法进行分组,各30例。对照组采取绿激光汽化治疗,观察组在对照组治疗基础上联合围手术期提肛肌训练。比较2组患者治疗效果和对勃起功能的影响。结果观察组术后6个月国际前列腺症状评分(international prostate symptom score,IPSS)为(4.82±1.23)分,低于对照组的(8.35±1.44)分;生活质量评分为(86.28±6.23)分,高于对照组的(71.54±5.49)分(P<0.05)。观察组术后1个月阴茎勃起次数、头部硬度≥60%持续时间、根部硬度≥60%持续时间、头部周径变化、根部周径变化等夜间勃起功能监测(nocturnal penile tumescence and rigidity,NPTR)参数分别为(2.98±0.61)次、(29.28±4.32)min、(30.62±4.24)min、(2.10±0.38)cm、(2.05±0.29)cm,均优于对照组的(2.62±0.24)次、(24.52±3.50)min、(23.98±4.61)min、(1.96±0.30)cm、(1.95±0.21)cm(P<0.05)。观察组不良反应总发生率为6.67%,低于对照组的26.67%(P<0.05)。结论绿激光汽化联合围手术期提肛肌训练治疗大体积BPH有显著疗效,不仅可以减少术后勃起功能障碍的风险,还能改善预后,降低其不良反应发生率。Objective To investigate the efficacy of green laser vaporization combined with perioperative levator anal training in the treatment of large volume benign prostatic hyperplasia(BPH).Methods A total of 60 patients with large volume BPH who were admitted to the department of urology,Dongguan Hospital of Integrated Traditional Chinese and Western Medicine from January 2022 to June 2023 were selected as the observation objects.Random number table method was used to group 30 cases each.The control group received green laser vaporization treatment,and the observation group was combined with perioperative levator anal training on the basis of the control group.The therapeutic effect and the effect on erectile function were compared between the two groups.Results Six months after operation,international prostate symptom score(IPSS)of the observation group was(4.82±1.23)points,lower than(8.35±1.44)points of the control group.The quality of life score was(86.28±6.23)points,higher than(71.54±5.49)points of the control group(P<0.05).One month after operation,the NPTR parameters of the observation group,such as the number of penile erection,the duration of head hardness≥60%,the duration of root hardness≥60%,the changes of head circumference diameter,and the changes of root circumference diameter were(2.98±0.61)times,(29.28±4.32)min,(30.62±4.24)min,(2.10±0.38)cm,(2.05±0.29)cm,which were better than(2.62±0.24)times,(24.52±3.50)min,(23.98±4.61)min,(1.96±0.30)cm,(1.95±0.21)cm of the control group(P<0.05).The total incidence of adverse reactions of the observation group was 6.67%,which was lower than 26.67%of the control group(P<0.05).Conclusion Green laser vaporization combined with perioperative levator anal training has significant effect in the treatment of large volume BPH,which can not only reduce the risk of postoperative erectile dysfunction,but also improve the prognosis and reduce the incidence of adverse reactions.
