摘　　要：糖皮质激素是治疗哮喘气道炎症的重要药物,但哮喘患者对糖皮质激素的反应各不相同,其中5%~10%的哮喘患者对大剂量吸入型糖皮质激反应较差,这被称为激素抵抗型哮喘(SRA)。研究表明,糖皮质激素受体(GR)在哮喘治疗中发挥着至关重要的作用,其通过与糖皮质激素结合并调节下游基因表达而控制哮喘的炎症反应,进而影响机体对糖皮质激素的反应。本文从GR角度综述了SRA的危险因素、发病机制及对策,以期为SRA的治疗提供新的思路和方法。Glucocorticoid is an important drug for the treatment of airway inflammation in asthma,but the response of asthma patients to glucocorticoids is different.Among them,5%to 10%of asthma patients have poor response to high-dose inhaled glucocorticoids,which is called steroid resistant asthma(SRA).Studies have shown that glucocorticoid receptor(GR)plays a crucial role in the treatment of asthma.It controls the inflammatory response of asthma by binding to glucocorticoids and regulating the expression of downstream genes,thereby affecting the body's response to glucocorticoids.This article reviews the risk factors,pathogenesis and countermeasures of SRA from the perspective of GR,in order to provide new ideas and methods for the treatment of SRA.
