摘 要:目的:探讨煤工尘肺患者神经元特异性烯醇化酶(NSE)、纤维蛋白原(FIB)等指标的表达水平及其在尘肺诊断与进展中的价值.方法:选取2022年1月至2023年12月河南省第三人民医院收治的119例煤工尘肺患者作为研究对象,纳入研究组;另选取同期80例健康体检者纳入对照组.比较两组患者NSE、FIB、白蛋白与球蛋白比值(A/G)、中性粒细胞与淋巴细胞比值(NLR),比较不同期别尘肺患者各项检测指标差异,分析各指标联合检测在尘肺病诊断中的价值.结果:研究组患者NSE、FIB、NLR均高于对照组,A/G低于对照组(P<0.05);Ⅲ期尘肺患者NSE、FIB、NLR高于Ⅰ期患者,且NSE高于Ⅱ期患者(P<0.05);NSE、FIB、A/G、NLR联合检测诊断尘肺病的曲线下面积(AUC)为0.893,灵敏度为0.840,特异度为0.812,均高于单一指标检测.结论:煤工尘肺体内NSE、FIB、A/G、NLR异常,且NSE、FIB、NLR与疾病分期有关,各指标联合检测可提高尘肺病检出率.Objective:To investigate the expression levels of neuronspecific enolase(NSE)and fibrinogen(FIB)and other indicators in coal workers'pneumoconiosis patients and their value in the diagnosis and progression of pneumoconiosis.Methods:A total of 119 coal worker's pneumoconiosis patients admitted to the third people's hospital of henan province from January 2022 to December 2023 were selected as the research subjects and included in study group.Another 80 healthy examinees in the same period were included in control group.The differences of NSE,FIB,albumin-to-globulin ratio(A/G),and neutrophil to lymphocyte ratio(NLR)between the two groups were compared,and the differences of various detection indicators of different stages of pneumoconiosis patients were compared,and the value of combined detection of all indicators in the diagnosis of pneumoconiosis was analyzed.Results:NSE,FIB and NLR in study group were higher than those in control group,and A/G was lower than that in control group,and the differences were statistically significant(P<0.05).The NSE,FIB and NLR of patients with stage II pneumoconiosis were higher than those with stage I pneumoconiosis,and the NSE was higher than those with stage I pneumoconiosis,and the differences were statistically significant(P<0.05).The area under the curve(AUC=0.893),sensitivity(0.840)and specificity(O.812)of the combined detection of NSE,FIB,A/G and NLR in the diagnosis of pneumoconiosis were all higher than that of single detection.Conclusion:Abnormal expression of NSE,FIB,NLR and A/G in coal workers'pneumoconiosis,and NSE,FIB and NLR are related to disease stage.Combined detection of all indexes can improve the detection rate of pneumoconiosis.
