艾灸联合中医情志护理对胃肠术后患者麻醉恢复期应激反应与并发症的影响

摘　　要：目的:探讨艾灸联合中医情志护理对胃肠术后患者麻醉恢复期应激反应与并发症的影响。方法:将胃肠手术患者62例随机分为对照组和观察组各31例。对照组实施常规护理,观察组在对照组基础上予艾灸联合中医情志护理,干预至出麻醉监测治疗室(postanesthesia care unit,PACU)时。比较两组患者应激反应指标、复苏情况及并发症发生率。结果:出PACU时两组患者血清去甲肾上腺素、肾上腺素、皮质醇水平均增加(P<0.01),观察组低于对照组(P<0.01);观察组患者麻醉苏醒、气管拔管、麻醉恢复室滞留时间均短于对照组(P<0.01);观察组患者恶心、呕吐、呼吸抑制、体动反应的总发生率低于对照组(P<0.05)。结论:在常规护理的基础上,加艾灸联合中医情志护理可有效降低胃肠术后患者的应激反应,缩短苏醒时间与麻醉恢复室滞留时间,降低并发症发生率。Objective:To survey the effects of moxibustion and TCM emotional care on stress response and complications in patients at recovery stage from anesthesia after gastrointestinal surgery.Methods:All 62 patients were randomized into the control group and the observation group with 31 cases in each group.The control group implemented conventional care,and the obsevation group was given moxibustion and TCM emotional care on the foundation of routine care,till the time of PACU discharge.To compare the indicators of stress response,the recovery situation and the incidences of the complications between both groups.Results:when discharging from PACU,the levels of serum norepinephrine,adrenaline and cortisol increased in the two groups(P<0.01),the observation group was lower than the control group(P<0.01);the observation group was lower than the control group in the times of anesthesia recovery,tracheal extubation and anesthesia recovery room stay(P<0.01);the overall incidences of nausea,vomiting,ventilatory depression and motor response of the observation group were lower than these of the control group(P<0.05).Conclusion:On the foundation of routine care,moxibustion and TCM emotional care could effectively relieve stress response in patients after gastrointestinal surgery,shorten recovery time and anesthesia recovery room stay,and lower the incidence of the complications.
