内踝骨折行关节镜下闭合复位内固定与切开复位内固定疗效的对比分析

摘　　要：目的探讨内踝骨折行关节镜下闭合复位内固定与切开复位内固定(ORIF)的对比疗效。方法选取晋江市医院(上海市第六人民医院福建医院)2021年10月-2022年10月就诊的内踝骨折患者43例,随机分为实验组和对照组,实验组23例采用关节镜下闭合复位内固定,平均年龄为(41.90±14.30)岁;对照组20例采用传统ORIF,平均年龄为(37.91±14.16)岁,随访12-18个月;对比两组患者的住院天数、手术时间与末次随访的美国足踝外科协会踝-后足功能(AOFAS)评分。结果住院天数实验组为(9.10±2.17)d,对照组为(13.13±6.96)d,两者差异具有统计学意义(P<0.05);对照组手术时间为(67.20±37.72)min,实验组为(100.05±24.45)min,两者差异具有统计学意义(P<0.05);末次随访的AOFAS评分,实验组与对照组比较差异有显著统计学意义(P<0.001)。结论内踝骨折行关节镜下闭合复位内固定对比ORIF手术治疗,可缩短患者住院时间,改善患者术后踝关节功能。Objective To compare the clinical efficacy of arthroscopic closed reduction internal fixation with Open Reduction Internal Fixation(ORIF)for medial malleolus fractures.Method A total of 43 patients with medial malleolus fractures treated at Jinjiang hospital between October 2021 and October 2022 were randomly divided into experimental group and control group.The experimental group(n=23)underwent arthroscopic closed reduction internal fixation,with an average age of(41.90±14.30)years,while the control group(n=20)underwent traditional ORIF,with an average age of(37.91±14.16)years.Both groups were followed up for 12 to 18 months.Hospital stay duration,operation time and the ankle-hindfoot scores by American Orthopaedic Foot and Ankle Society(AOFAS)at the final follow-up were compared between the two groups.Results The experimental group had a significantly shorter hospital stay(9.10±2.17 days)compared to the control group(13.13±6.96 days),and the difference was statistically significant(P<0.05).The control group had a shorter operation time(67.20±37.72 minutes)than the experimental group(100.05±24.45 minutes),with a statistically significant difference(P<0.05).In terms of the AOFAS scores at the final follow-up,the difference between the experimental group and the control group was statistically significant(P<0.001).Conclusion Compared with ORIF,arthroscopic closed reduction internal fixation for medial malleolus fractures can shorten the hospital stay and improve postoperative ankle joint function.
