基于数据挖掘探讨中药保留灌肠治疗慢性直肠炎的用药规律

摘　　要：目的:借助数据挖掘技术分析中药保留灌肠治疗慢性直肠炎的用药规律。方法:检索中国知网、万方、维普中文医学数据库,收集整理自建库至2023年12月31日中药保留灌肠治疗慢性直肠炎的期刊文献,筛选出临床病例观察的文献,使用Excel(2021)表格建立中药灌肠方数据库,对纳入文献所涉及的病例数以及出现的药物频数、性味、归经等进行统计分析,通过IBM SPSS Modeler 18.0、IBM SPSS Statistics 26软件对高频药物进行聚类分析、关联规则、复杂网络等多方位分析。结果:检索主题相关文献181篇,逐篇阅读后筛选出有效灌肠处方58首,药物共计115味,用药总频次512次,其中高频药物(使用频次≥7)24味,前7味分别是黄连、地榆、白及、败酱草、黄柏、当归和黄芩;药性以寒、温、平为主;药味以苦、甘、辛为主;药物归经以肝、肺经为主,其次为脾经、胃经、大肠经、心经,功效以清热利湿解毒、调气行血导滞为主。使用频次前5位的药对为黄连-黄芩、地榆-败酱草、地榆-白及、黄连-黄柏、地榆-槐花,聚类分析得出核心药物组合5个。结论:保留灌肠方治疗慢性直肠炎大多寒温并用,苦辛为法,补泻兼施,肝脾肺胃同调,用药以温阳化湿药、清热燥湿药、泻火解毒药、凉血止血药、理气健脾药为主。Objective To analyze the medication regularity of Chinese medicinal retention enema in the treatment of chronic proctitis by means of data mining technology.Methods By searching,collecting and sorting out the journal literature of Chinese medicinal retention enema in the treatment of chronic proctitis in the medical database of CNKI,Wanfang and VIP from the establishment to December 31,2023,the literature of clinical case observation was screened out,and the database of Chinese medicinal enema formulas was established using Excel(2021).Statistical analysis was carried out on the number of cases involved in the included literature,as well as the drug frequency,property,flavor and channel tropism,etc.IBM SPSS Modeler 18.0 and IBM SPSS Statistics 26 were used to conduct multi-dimensional analysis of high-frequency drugs such as cluster analysis,association rules and complex networks.Results A total of 181 literatures related to the subject were searched and 58 effective enema formulas were selected after reading one by one,with a total of 115 drugs and a total frequency of 512.Among them,24 drugs were high-frequency drugs(frequency≥7),and the top 7 drugs were Huanglian(Coptidis Rhizoma),Diyu(Sanguisorbae Radix),Baiji(Bletillae Rhizoma),Baijiangcao(Patrinia Herba),Huangbo(Phellodendri Chinensis Cortex),Danggui(Angelicae Sinensis Radix)and Huangqin(Scutellariae Radix).The medicinal property was mainly cold,warm or neutral.The medicinal flavor was mainly bitter,sweet or acrid.The channel tropism was mainly liver or lung channel,followed by spleen,stomach,large intestine or heart channel,with the main effect of clearing heat,draining dampness,removing toxin,regulating qi and eliminating stagnation.The top 5 frequently used medicinal pairs were Huanglian-Huangqin,Diyu-Baijiangcao,Diyu-Baiji,Huanglian-Huangbo,and Diyu-Huaihua(Sophorae Flos),and five core medicinal combinations were obtained by cluster analysis.Conclusion Retention enema formulas in the treatment of chronic proctitis mostly use both cold and warm drugs,a
