腹腔镜下保留睾丸动脉精索静脉高位结扎术联合生精片治疗精索静脉曲张不育患者的临床疗效分析

摘　　要：目的分析腹腔镜下保留睾丸动脉精索静脉高位结扎术联合生精片治疗精索静脉曲张不育患者的临床疗效.方法选择湛江中心人民医院泌尿外科2020年-2021年86例双侧精索静脉曲张患者,随机分为:实验组A(32例):腹腔镜下保留睾丸动脉高位精索血管集束结扎术(Ivanis-sevich)+生精片、对照组B(28例):Ivanis-sevich术、对照组C(26例):高位精索血管集束结扎术(Palomo),分析三组精液质量、性激素水平、疼痛程度及复发率.结果与术前相比,三组术后各项精液质量指标均明显上升,且A组比B组、C组更高,B组比C组更好(P<0.05);三组术后T水平明显上升,LH水平和疼痛程度明显下降(P<0.05);A组、B组均无复发,C组仅有1例(3.85%)复发,三组间比较,差异无统计学意义(P>0.05).结论腹腔镜下保留睾丸动脉精索静脉高位结扎术联合生精片治疗效果更为显著,可明显改善精液质量和性激素水平,且疼痛较轻,复发率更低,临床应用价值较高.Objective To analyze the clinical efficacy of laparoscopic high ligation of testicular artery and spermatic vein combined with spermatogenic tablets in the treatment of infertile patients with varicocele.Methods 86 patients with bilateral varicocele in the urology department of our hospital from 2020 to 2021 were selected and randomly divided into:experimental group A(32 cases):laparoscopic preservation of testicular artery high spermatic vein bundle ligation(Ivanis evich)+spermatogenic tablets;control group B(28 cases):Ivanis evich surgery;control group C(26 cases):high spermatic vein bundle ligation(Palomo).Semen quality,sex hormone levels,pain level,and recurrence rate were analyzed in the three groups.Results Compared with preoperative,all three groups showed a significant increase in postoperative semen quality indicators,and group A was higher than group B and group C,while group B was better than group C with P<0.05;the postoperative T levels in the three groups significantly increased,while LH levels and pain levels significantly decreased(P<0.05);there was no recurrence in group A and group B,while only one case(3.85%)in group C experienced recurrence.There was no statistically significant difference between the three groups(P>0.05).Conclusion Laparoscopic preservation of testicular artery and spermatic vein high ligation combined with spermatogenic tablets has a more significant therapeutic effect,can significantly improve semen quality and sex hormone levels,and has less pain,lower recurrence rate,and higher clinical application value.
