探讨颅内动脉瘤患者在介入术后发生脑血管痉挛的影响因素及护理对策分析

摘　　要：目的分析颅内动脉瘤(IA)患者介入术后脑血管痉挛发生的影响因素与护理对策.方法回顾性分析2021年9月-2023年9月中山市人民医院收治的103例行介入术的IA患者的临床资料,统计其术后脑血管痉挛发生率;另收集患者的年龄、性别等资料,探究影响IA患者介入术后脑血管痉挛发生的有关因素.结果103例IA患者,介入术后共有32例(31.07%)发生脑血管痉挛;单因素分析显示:年龄、Fisher分级、Hunt-Hess分级、动脉瘤大小、手术时机、血糖水平与IA患者介入术后脑血管痉挛的发生有关(P<0.05);Logistic回归分析结果显示:年龄>60岁(OR=5.172,95%CI=1.976-13.542)、Fisher分级>Ⅱ级(OR=8.503,95%CI=3.189-22.678)、Hunt-Hess分级>Ⅱ级(OR=3.925,95%CI=1.586-9.709)、动脉瘤大小≥1cm(OR=6.655,95%CI=2.431-18.221)、入院>3d行手术(OR=3.808,95%CI=1.564-9.271)、血糖水平>7.0mmol/L(OR=3.111,95%CI=1.300-7.447)是IA患者介入术后脑血管痉挛发生的独立危险因素(P<0.05).结论年龄>60岁、Fisher分级>Ⅱ级、Hunt-Hess分级>Ⅱ级、动脉瘤大小≥1cm、入院>3d行手术、血糖水平>7.0mmol/L是IA患者介入术后脑血管痉挛发生的独立危险因素.Objective To analyze the influencing factors and nursing strategies of cerebral vasospasm in patients with intracranial aneurysm(IA)after interventional surgery.Methods A retrospective analysis was conducted on the clinical data of 103 IA patients who underwent interventional surgery in our hospital from September 2021 to September 2023,and the incidence of postoperative cerebral vasospasm was calculated;collect information on the age,gender,and other factors of patients to explore the factors affecting the occurrence of cerebral vasospasm in IA patients after intervention surgery.Results Among the 103 IA patients,a total of 32(31.07%)experienced cerebral vasospasm after intervention surgery;univariate analysis showed that age,Fisher grade,Hunt-Hess grade,aneurysm size,surgical timing,and blood glucose levels were associated with the occurrence of cerebral vasospasm in IA patients after intervention(P<0.05);the results of Logistic regression analysis showed that age>60 years old(OR=5.172,95%CI=1.976-13.542),Fisher's grade>GradeⅡ(OR=8.503,95%CI=3.189-22.678),Hunt-Hess grade>GradeⅡ(OR=3.925,95%CI=1.586-9.709),aneurysm size≥1cm(OR=6.655,95%CI=2.431-18.221),admission>3d surgery(OR=3.808,95%CI=1.564-9.271),blood glucose>7.0mmol/L(OR=3.111,95%CI=1.300-7.447)is an independent risk factor for postoperative cerebral vasospasm in IA patients(P<0.05).Conclusion Age>60 years old,Fisher's grade>GradeⅡ,Hunt-Hess grade>GradeⅡ,aneurysm size≥1cm,admission>3 days of surgery,and blood glucose>7.0mmol/L are independent risk factors for postoperative cerebral vasospasm in IA patients.
