全结肠系膜切除术治疗右半结肠癌的效果研究

摘　　要：目的探讨全结肠系膜切除术(CME)治疗右半结肠癌的效果,为右半结肠癌患者的治疗提供参考。方法选取2018年2月—2020年11月青岛市城阳区人民医院收治的右半结肠癌患者87例,按治疗方式分为对照组39例和研究组48例。对照组采用传统非全结肠系膜切除术治疗,研究组采用CME治疗。比较两组手术时间、术中出血量、术后排气时间、淋巴结清扫数量、术后1 d、3 d血浆白蛋白及前白蛋白水平以及术后并发症发生率。结果研究组手术时间、术中出血量与对照组比较,差异均无统计学意义(P>0.05);研究组术后排气时间、清扫淋巴结数量均优于对照组,差异均有统计学意义(P<0.05)。研究组术后1 d、3 d血浆白蛋白及前白蛋白水平均高于对照组,差异均有统计学意义(P<0.05)。研究组并发症发生率为6.25%,低于对照组的25.64%,差异有统计学意义(χ^(2)=6.366,P<0.05)。结论CME相比传统手术,并没有增加手术时间及手术出血量,而缩短了术后恢复排气时间,使淋巴结清扫更彻底,对机体营养指标的影响更小,且术后并发症发生率更低,因此CME的治疗效果理想,值得在临床上推广应用。Objective To explore the effect of complete mesocolic excision(CME)in the treatment of rightsided colon cancer,in order to provide reference for the treatment of the patients with right-sided colon cancer.Methods A total of 87 patients with right-sided colon cancer admitted to Qingdao Chengyang District People's Hospital from February 2018 to November 2020 were selected and divided into control group with 39 cases and study group with 48 cases according to treatment methods.The control group was treated with traditonal non complete mesocolic excision,the study group was treated with CME.The operation time,intraoperative bleeding,postoperative exhaust time,number of lymph node dissection,the level of plasma albumin and prealbumin on the first and third day after operation and the incidence of postoperative complications were compared between the two groups.Results There were no statistically significant differences between the study group and the control group in the operation time,intraoperative bleeding volume(P>0.05).The exhaust time and the number of lymph nodes dissection in the study group were better than those in the control group,and the differences were statistically significant(P<0.05).The plasma albumin and prealbumin levels of the study group on the first day and third day after operation were higher than those of the control group,and the differences were statistically significant(P<0.05).The incidence of complications in the study group was 6.25%,which was lower than 25.64%in the control group,and the difference was statistically significant(χ^(2)=6.366,P<0.05).Conclusion Compared with traditional surgery,CME did not increase the operation time and the amount of surgical bleeding,while shorted the postoperative recovery time of exhaust,the lymph node cleaning was more thorough,which had less impact on the nutritional indicators of the body,and the incidence of postoperative complications was lower.Therefore,CME had an ideal therapeutic effect and was worth promoting and applying in clinical practic
