摘  要:类风湿关节炎(rheumatoid arthritis,RA)是一种自身免疫异常介导的慢性炎症性关节疾病,其特点是滑膜炎症和不可逆的骨侵蚀,与骨免疫系统功能异常有关。骨丢失是RA的典型后果,局部骨丢失能够导致发炎关节产生糜烂性改变,而全身性骨丢失会继发骨质减少或明显的骨质疏松(osteoporosis,OP)。骨免疫学被定义为一个新的交叉学科领域,涉及了包括破骨细胞、成骨细胞、免疫细胞等在内的骨骼系统和免疫系统的相互作用。本文通过查阅相关国内外文献,综述了在骨免疫系统下目前RA继发OP的病理生理学方面取得的实质性进展,以期为预防RA及抗OP提供参考。Rheumatoid arthritis(RA)is a chronic inflammatory joint disease mediated by autoimmune abnormalities,characterized by synovial inflammation and irreversible bone erosion,which is associated with abnormal bone immune system function.Bone loss is a typical consequence of RA.Local bone loss can lead to erosive changes in inflamed joints,while systemic bone loss can lead to secondary osteopenia or obvious osteoporosis(OP).Osteoimmunology is defined as a new interdisciplinary field involving the interaction between the skeletal system and the immune system,including osteoclasts,osteoblasts,immune cells,etc.This article reviews relevant domestic and foreign literature and reviews the substantial progress made in the pathophysiology of OP secondary to RA based on the bone immune system,in order to provide reference for the prevention of RA and anti OP.
